Introduction: Patients with CKD have an impaired health-related quality of life (QoL). Most studies have been conducted on dialysis patients, and less is known about QoL and its determinants in predialysis patients. We studied the association between QoL and comorbidities, cardiac biomarkers, echocardiography, and mortality in patients with CKD stage 4–5 not on dialysis. Methods: A total of 140 patients enrolled in the Chronic Arterial Disease, Quality of Life and Mortality in Chronic Kidney Injury (CADKID) study filled the Kidney Disease Quality of Life Short Form (KDQOL-SF) at the beginning of the study. Echocardiography and biochemical parameters were obtained at baseline. Patients were followed up for at least 2 years or until death. Results: The median age was 66 years, and 51 (36%) patients were female. The median estimated glomerular filtration rate was 13 mL/min per 1.73 m2. Obesity, diabetes, atrial fibrillation, and congestive heart failure were associated with lower QoL scores in multiple KDQOL-SF domains. Cardiac biomarkers, troponin T (p = 0.02), N-terminal pro-B-type natriuretic peptide (p = 0.006), and the echocardiographic parameter of cardiac systolic function left ventricular global longitudinal strain (p = 0.02) were significant predictors of lower physical component summary (PCS) score in multivariable regression models after controlling for age, BMI, and diabetes. A low PCS score predicted mortality in a multivariable Cox proportional hazards model [HR 0.96 (95% CI 0.92–0.99), p = 0.03]. QoL was not associated with kidney disease progression. Conclusion: Impaired QoL in CKD stage 4–5 patients not on dialysis is associated with cardiac biomarker levels, echocardiographic indices, and mortality.

Background: Anthracycline-induced cardiotoxicity is associated with a poor prognosis. We can use Global longitudinal strain (GLS) to detect subclinical left ventricular dysfunction in chemotherapy patient. The distance of 6-minute walk test (6MWT) has been used as a prognostic factor in cancer patients.This study aimed to prove whether there is a correlation between left ventricular dysfunction before there are clinical signs of heart failure and decreased functional capacity of 6MWT in breast cancer patients. Methods: This is a cohort prospective study of 35breast cancer patients who have chemotherapy with anthracycline regimen. The patients were evaluated before the initiation of anthracycline therapy, after 3 cycles and 6 cycles. Patients underwent standard echocardiography and 6MWT at the first visit. After 3 cycle patient underwent echocardiography and after 6 cycle patient underwent echocardiography and 6MWT. Results: The mean age was 45.83 ± 6.96 years. There was a significant difference in percentage reduction of GLS after chemotherapy 3 cycles and 6 cycles (10.49 ± 4.94 vs. 20.717 ± 9.616, p = 0.001). GLS is correlated with a decrease in total distance 6MWT after 6 cycles (R 2 =-0.084) and the correlation of percentage reduction of GLS and distance of 6MWT has p = 0.028 and R 2 =-0.112. Conclusion: There is a correlation between the percentage reduction of GLS and distance of 6MWT in breast cancer patients with anthracycline chemotherapy for 6 cycles (R 2 =-0.112).

Patients with chronic kidney disease show poorer functional and cardiorespiratory capacity than healthy individuals, and these impairments result in sedentarism. The aim of this study was to conduct a systematic review and meta-analysis of randomized clinical trials on the effects of different intradialytic exercise protocols on cardiopulmonary capacity in chronic kidney disease patients. The primary outcome was peak oxygen consumption (VO2peak) and the secondary outcomes were exercise duration and ventilation in the cardiopulmonary test. The quality of the evidence was evaluated using the GRADE guidelines. Seven studies with a total of 124 participants met the inclusion criteria. Compared to the non-exercise group, the exercise group improved in mean VO2peak (MD 4.06 [IC 0.81; 7.31]). In a separate analysis according to exercise modality, aerobic exercise plus strength training performed better than aerobic exercise alone (MD 5.28 [IC 3.90; 6.66]). In the exercise group, both exercise tolerance values (MD 3.10 [IC 1.70; 4.51]) and ventilation values in the cardiopulmonary test were better than those of the control group (MD 13.10 [IC 7.12; 19.09]). Thus, intradialytic exercise protocols can improve cardiopulmonary function, exercise tolerance and ventilatory efficiency in chronic kidney disease patients.
