Epidemiological evaluation of apical periodontitis prevalence in an urban Brazilian population

The present study aimed to assess the prevalence of apical periodontitis (AP) in an urban Brazilian population according to gender, age group and tooth type. Data were collected from clinical files containing the medical and dental histories and periapical radiographs of 1,126 patients treated at the School of Dentistry at Universidade do Estado do Rio de Janeiro between March 2000 and December 2010. A total of 15,724 periapical radiographs were evaluated. All the radiographs were evaluated by two independent, previously calibrated endodontists (kappa = 0.88). Periapical areas on the radiographs were classified as N (normal) or AR (apical radiolucency). The frequency of AP and the 95% Confidence Interval (95%CI) were calculated according to gender, age group and tooth type. Differences between groups were calculated using the Z-test at a significance level of 5% (p < 0.05). AP was present in 7.87% of the samples, with 16.70% occurring on previously endodontically treated teeth and 44.65% occurring on teeth referred for endodontic treatment (TR-RCT). The frequency of AP was higher among females (64%) than among males (35%). The central and lateral maxillary incisors were the most frequently affected teeth. The frequency of AP was higher among individuals between 30 and 49 years of age. In this population, AP was more prevalent among females and among individuals between 30 and 49 years of age, and the central and lateral maxillary incisors were the most frequently affected teeth.

ResumoHá décadas tem se estudado a associação de doenças orais e sistêmicas, embora a relação de causa e efeito ainda não tenha sido claramente estabelecida. Com o ressurgimento da teoria da infecção focal, outros estudos analisaram a relação entre a existência do foco infeccioso de natureza bacteriana com a possibilidade de disseminação, o que pode condicionar a doenças sistêmicas, como, por exemplo, a endocardite bacteriana. Nesse sentido, a verificação da disseminação microbiana como fator de risco para doenças cardíacas tem sido investigada em vários estudos. Análises epidemiológicas demonstraram a relação entre a doença periodontal crônica e doença cardíaca coronariana, acidente vascular cerebral, prematuridade e baixo peso ao nascimento e doença respiratória. Entretanto, o papel da periodontite apical (PA) associada a infecções endodônticas no desenvolvimento de alterações sistêmicas tem sido alvo de poucos estudos. As infecções periodontais e as infecções endodônticas têm uma microbiologia complexa e estão associadas a níveis elevados de citocinas. O objetivo deste trabalho foi caracterizar a produção científica publicada nos últimos 5 anos relacionando PA com doenças cardiovasculares (DVC) e descobrir quais as principais alterações cardíacas ligados aos estudos de PA. In this sense, the verification of microbial spread as a risk factor for heart disease has been investigated in several studies. Epidemiological analyzes have demonstrated the relationship between chronic periodontal disease and coronary heart disease, stroke, prematurity and low birth weight and respiratory disease. However, the role of associated apical periodontitis (AP) to endodontic infections in the development of systemic alterations has been the subject of few studies. Periodontal infections and endodontic infections have a complex microbiology and are associated with elevated levels of cytokines. The aim of this study was to characterize the scientific production published in the last 5 years relating AP to CVD cardiovascular diseases and to discover the main cardiac alterations related to AP studies. Aim: Periodontitis is caused by chronic gingival inflammation and affects a large population in the world. Although guided tissue regeneration (GTR) therapy has been proven to be an effective treatment, the deficiency in the symmetrical design of all the GTR membrane in the market leaves large space for improvement. Therefore, we designed a novel asymmetrical bi-layer PLA/gelatin composite membrane for treating periodontitis. Methods: The PLA side was fabricated by electrospinning with metronidazole (MNA) pre-mixed with the PLA solution. The gelatin side containing bioglass (BG) 45S5 was fabricated with freeze-drying process and cross-linked with PLA membrane. The bio-compatibility of the membrane was evaluated in vitro using NIH3T3 cells. The releasing of MNA was measured by spectrophotometer. The bioactivity of the membrane was evaluated by hydroxyapatite (HA) deposit and determined by FTIR spectrometer. The ionic concentration of Ca 2+ and 2 4 SiO − was measured by ICPOES.The expression of the osteogenesis makers was determined by qRT-PCR. Results: The bi-layer PLA/gelatin composite membrane is biocompatible and bioactive. The releasing of MNA can rapidly reach the anti-bacterial effective concentration. Interestingly, the incorporation of MNA modulated the degradation rate of PLA scaffold to meet the requirement of tissue regeneration. Meanwhile, the embedding of the BG powder in the gelatin porous layer provided a favorable Ca 2+ and 2 4SiO − ion environment for the regeneration of the alveolar bone tissue. Conclusions: Taken together, this bi-layer GTR membrane is closer to the physiological structure of the periodontal. The addition of MNA and BG makes it more powerful in treating periodontitis. Moreover, this research provides an example of biomimetic design in fabricating biomaterial for clinical applications.
