Recurrent aphthous stomatitis is one of the most common oral mucosal diseases seen by dental professionals, and yet its aetiology remains unclear, and its management based on less than robust evidence. The literature remains confused because of the lack of clarity in diagnosis and the lack of a standardised ulcer severity scoring system and agreed outcome measures. However, recent literature is encouraging in meeting these aims. There is agreement that recurrent aphthous stomatitis (RAS) is a localized mucosal disease not secondary to systemic disease and therefore distinguishable from over 40 other types of oral ulcers. Disease severity scores have been introduced and outcome measures have become more standardised. RAS appears to be an autoimmune disease directed at epitopes of heat shock proteins whilst most recent work on aetiology has focused on cytokines and genetics. Pro-inflammatory cytokines including TNF-a and IL-6 and IL-17 are raised in RAS and TNF inhibitors can inhibit episodes of ulceration. Many local antiinflammatory agents will help ulcers heal, and local steroids remain the treatment of choice. Some systemic drugs have evidence-based data indicating efficacy at preventing new ulcers including colchicine, prednisolone, thalidomide, pentoxyfilline and dapsone. The field would benefit from further trials combining local and systemic therapy using defined outcome measures.

The use of herbal medicine is increasingly becoming more accepted in treatment of many inflammatory conditions. Omega-3 is recently introduced as an effective therapeutic agent and a powerful anti-inflammatory agent, which promotes whole-body wound healing including ulcers as it has an important role in modulation of proinflammatory cytokine production. This study was conductedto evaluate and compare the effect of topicalapplication and systemic administration of omega-3 on the healing of oral mucosal wounds which wereexperimentally induced in albino rats. The experiment comprised 45 albino rats which were divided into 3 groups (n= 15 per group); control group not receiving any treatment(C) group (L) receiving topical application of omega3, and group(S) receiving systemic omega3; 5 rats from each group were sacrificed at day 2, 4 and 8 respectively. Tissue samples were obtained and prepared for histological and biochemical evaluations. The data obtained were analyzed using one way ANOVA and Post Hoc Tukey statistical analysis with the previously performed tests of normality and homogeneity. The biochemical results showed significant improvement regarding TNF-α and IL8-levels after 4 and 8 days in both groups compared to the control. The topical omega3 group showed better results than the systemic with significant differences at 4& 8 days inTNF-α while for IL8 the difference was only significantafter 8 days. The histological results revealed severe epithelial degeneration and inflammation in the lamina propria after 2 days whether using local or systemic treatment however increase reepithelialization gradually increased at 4 days and an even thickness of the epitheliumwas noted by the end of day 8. Conclusion:Omega 3 greatly enhanced ulcer healing and showed superior effects using topical application.

Interleukin as Biomarker of Recurrent Aphthous Stomatitis (Ras): A Systematic Literature Review

This review aimed to describe the interleukins and interleukin gene polymorphisms related to and recommended as a RAS biomarker. Articles were searched through PubMed, ScienceDirect, and Cochrane Library databases, using the keywords of “Interleukin” AND “Recurrent Aphthous Stomatitis”. The Risk of Bias Assessment tool for Non-randomized Studies (RoBANS) was used, and the writing of this review refers to the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines. A total of 8 articles met the criteria and showed a low risk of bias assessment. The level of IL-2, IL-6, IL-8, and IL-18 in the acute clinical phase of RAS were higher than in the recovery phase, but IL-10 levels showed decreased. IL-2, IL-6, IL-10 gene polymorphisms were found to be more frequent in RAS patients compared to controls, while IL-12 gene polymorphisms were found to be less associated with RAS pathogenesis. Interleukins at the proteomic level that recommended as a pro-inflammatory biomarker are IL-2, IL-6, IL-8, IL-12, and IL-18, while an anti-inflammatory is IL-10. Only IL-2 can be recommended as a biomarker at the genomic level, as other interleukins still require more investigation.
