ÖzetAmaç: Açık kalp cerrahisi sonrası görülen komplikasyonlar çoğunlukla kardiyopulmoner baypas sırasında gelişen iskemi reperfüzyon hasarı ile ilişkilidir. Lidokain iskemi ve reperfüzyon dönemlerinde hücre içi sodyum ve kalsiyum kanallarını bloke eder ve hücre membranını reaktif oksijen metabolitlerine karşı korumaktadır. Bu çalışmada, kardiyopleji solüsyonuna lidokain eklenerek miyokardiyal iskemi reperfüzyon hasarına etkisi araştırıldı. Gereç ve Yön-tem: Eylül 2005 ve Nisan 2006 tarihleri arasında kliniğimizde elektif koroner arter baypas cerrahisi uygulanan 36 hasta araştırıldı. Hastalar iki gruba ayrıl-dı. Aortik kros klemp süresi boyunca kullanılan kan kardiyoplejisi içine çalışma grubundaki hastalarda (Grup I) 2 mg/kg lidokain eklenirken, kontrol grubundaki hastalarda (Grup II) ise standart kan kardiyoplejisi kullanıldı. İki grup hastada postop 6. ve 24. saatteki kardiyak enzim seviyeleri, inotropik destek ihtiyacı ve postoperatif atriyal fibrilasyon insidansı karşılaştırıldı. Bulgular: Çalış-maya yaş ortalaması 63(±5,5), yaş dağılımı 50-70 yıl olan ve ventrikül fonksiyonları bozulmamış (EF>%40) 36 hasta (13'ü kadın, 23'ü erkek) dahil edildi. İki grup arasında demografik veriler açısından anlamlı farklılık yoktu. İki grup arasında postop 6. ve 24. saatteki troponin I ve CK-MB değerleri, inotropik destek veya defibrilasyon ihtiyacı ve postoperatif atrial fibrilasyon insidansı bakımından anlamlı fark tespit edilemedi. Tartışma: Kardiyopleji solusyonuna 2 mg/kg dozunda lidokain ilave edilmesinin kardiyak enzim seviyeleri, inotropik destek ihtiyacı ve postoperatif atrial fibrilasyon insidansını etkilemez ve iskemi-reperfüzyon hasarının önlenmesinde katkısı yoktur.

In ischemia and reperfusion periods lidocaine blocks intracelluler sodium and calcium channels and protect cell membrane against reactive oxygen metabolites. In this study, lidocaine added to cardioplegia solution and its effects on myocardial ischemia-reperfusion injury was examined. Material and Method: 36 patients who underwent elective coronary artery bypass surgery in our clinic between September 2005 and April 2006 was studied. Patients included into two groups. In study group patients (groupe I) 2 mg/ kg lidocaine was added into cardioplegia solution that is used during aortic cross clamp period; standart cardioplegia solution was used in control group patients (group II). Postoperative 6. and 24. hours cardiac enzyme levels, inotropic support requirement and atrial fibrilation incidence were compared in both groups. Results: In this study, 36 patients (13 women, 23 man) whose average age was 63(±5,5), age range 50-70 years and ventriculer functions were not deformed (EF>40%) were involved.
