Objectives Implants are used to replace congenitally missing lateral incisors but often the space across the alveolar crest is too narrow to permit their use. This multicenter study (Dental Clinic of the University of Foggia, Odontostomatology Clinic of the University of L’Aquila) evaluated the efficacy of mini-implants in cases of maxillary lateral incisor agenesis with severe osseous atrophy in 10-year follow-up. Materials and methods Forty-seven mini-implants have been inserted in 35 patients affected by lateral incisors agenesis (23 single and 12 bilateral ageneses). All patients underwent orthodontic opening of the space of the upper lateral incisors. After the insertion of the implants, the immediate, non-functional loading, positioning of crowns, presence of pain during percussion and mini-implant function, horizontal and vertical movement when a force of 5 N was applied, ridge loss, and plaque index have been evaluated 1 month after loading, 1 year after loading, and then every 5 years in the following 10 years. Little’s test was used to evaluate the assumption that data of loss to follow-up implants are missing completely at random (MCAR) and that a complete-case scenario could be adopted. Wilcoxon test was carried out to look statistically significant differences between the various parameters resulting in the complete-case scenario and those assumed for the worst scenario. The software R (v. 3.6.1, 2019) was employed to perform the statistical analysis. Results The results obtained over 10 years range from 89% of success rate in a worst-case scenario to the 100% using a complete-case analysis with satisfactory values of marginal bone resorption and good conditions of the peri-implant tissue. Ten-year follow-up using complete-case analysis shows survival rates of 100% for implants with no signs of peri-implantitis, stability of the marginal bone levels and soft tissue around the dental implants. Conclusions The data collected show very good implant stability, absence of progressive peri-implantitis, and satisfactory aesthetical results in time (no signs of infraocclusion). Clinical relevance Mini-implants can be considered a valid and stable over time solution in the restorative treatment of maxillary lateral incisors agenesis.

Introduction. Treatment of the hypodontia of the upper lateral incisors could be orthodontic or multidisciplinary with combined orthodontic, surgical and prosthetic involvement. Both options have their pros and cons and could be quiet challenging to manage, in particularly in cases of unilateral missing of upper lateral incisor in adult patients. The aim of this case report was to described two cases with these opposite treatment approaches in the young adult patients with unilateral missing of upper lateral incisors. Case report. First case presents combined orthodontic, surgical and prosthetic treatment of the missing upper right lateral incisor in the adult patient. In 22 years, old girl clinical examination showed missing tooth 12 with almost completely closed space, midline deviation, reduced overjet and overbite and Class III molar relationship on the right side and Class I molar relationship on the left side with V shape maxillary arch and cross bite tendency in the frontal region. Based on the skeletal Class III relationship and dental findings it was decided to open the space for the tooth 12 and establish overjet and overbite and also Class I occlusion. Surgical implant insertion followed orthodontic preparation, with crown positioning after surgical healing. Second case described orthodontic treatment of unilateral hypodontia in young adult patient. In 24 years old female clinical and radiographic examination revealed hypodontia of tooth 12 with atypical tooth 22 with severe crowding in the lower arch and Class I molar relationship on the right side and half Class II relationship on the left side. Treatment decision was to extract atypical tooth 22 and tooth 35 and 44, and to move upper teeth forward to close the space. After orthodontic treatment upper canines replaced missing lateral incisors. Conclusion: Both treatments successfully resolved malocclusion and obtained good esthetical and functional results. Treatment plan and decision to open or close the space in case of hypodontia should be made individually for each patient, according to his age, malocclusion, canines shape and size and patient preferences.

La agenesia de incisivos laterales superiores corresponde a la más común de las ausencias dentarias congénitas, después de los terceros molares. Trae consigo un compromiso estético y funcional, que necesita de un manejo multidisciplinario (1) . La prevalencia corresponde a un 0.8-2% de la población en general (2) .Considerando que el diagnóstico y el tratamiento suele ser en edad de crecimiento y desarrollo craneofacial, las alternativas terapéuticas para el reemplazo o sustitución de los dientes ausentes siguen aún en controversia (3) . Dentro de las alternativas terapéuticas para la sustitución de estos dientes, está el cierre de espacios mediante ortodoncia, con el reemplazo del incisivo lateral faltante por el canino adyacente; apertura de espacio para la colocación de un implante óseo integrado y su posterior rehabilitación; autotrasplante dentario y restauración con pónticos adheridos a las piezas dentarias adyacentes (4) . Cada una de estas alternativas busca éxito a largo plazo, junto con estética y función aceptable (5) .El reemplazo del incisivo lateral faltante por el canino adyacente mediante movimientos de ortodoncia, pareciera ser la opción más conservadora, con buenos resultados y estabilidad a largo plazo (2,6) . Sin embargo, requiere de restauraciones que desgastan estructura dental sana, con el fin de recuperar la armonía y estética del arco dentario (2) .En cuanto a la apertura de espacios y posterior instalación de implantes oseointegrados, es necesario tener en cuenta la edad del paciente, debido al riesgo de infraoclusión, sin embargo, se habla de que la aparición de esta es independiente de la edad e inevitable. Esto se explica debido a que el implante es incapaz de seguir la dirección y velocidad de crecimiento dentoalveolar de las piezas dentarias adyacentes. Además, se han observado alteraciones en relación a los tejidos blandos, comprometiendo la estética de la restauración (6) . REVISIÓN NARRATIVAAusencia congénita de incisivos laterales superiores.
