Assessment of Atrophic Gastritis Using the OLGA System

Abstract: Our results indicate that higher stages are found in patients with GC using the OLGA staging system and that the high risk of GC can be recognized. It is simple to use and useful for the assessment of the severity of atrophic gastritis.

“…35 In contrast, the consistent association of GC only with OLGA stages III ⁄ IV identifies this (low-prevalence) population as an elective target for endoscopic surveillance. 16,17 In this long-term follow-up study, the prognostic impact of OLGA-staging was tested in a consecutive series of dyspeptic outpatients. Both at enrolment (T1) and at the end of the follow-up (T2), a vast majority of the patients (89% and 86%, respectively) featured low-stage (0-I-II) gastritis.…”

“…24 High OLGA stages (III, IV) were reported to have an association with a high risk of GC. 25,26 Randomisation, surgery and follow-up Patients were randomly assigned to one of two parallel groups with an equal allocation ratio. H. pylori-positive GC patients who were scheduled to have subtotal or laparoscopy-assisted distal gastrectomy were randomised to receive a 7-day course of rabeprazole 10 mg, clarithromycin 500 mg and amoxicillin 1000 mg, all twice daily (n = 95); or placebo (n = 95) before surgery.…”

“…However, the prediction of high gastric cancer risk cannot be done using this classification system. The OLGA staging system has the capability to differentiate the high-risk cancer population (stages III/IV) from low risk gastric cancer population (stages 0/I/II) [13,20,21]. The low reported inter-observer agreement of the gastric atrophy staging [11] conducted to the proposal of a new staging system based on the assessment of severity and extension of intestinal metaplasia [12].…”
