Assessment of Antibiotic Treatment of Cellulitis and Erysipelas

Abstract: IMPORTANCE The optimum antibiotic treatment for cellulitis and erysipelas lacks consensus. The available trial data do not demonstrate the superiority of any agent, and data are limited on the most appropriate route of administration or duration of therapy. OBJECTIVE To assess the efficacy and safety of antibiotic therapy for non-surgically acquired cellulitis.

“…The treatment should be directed against typical Gram-positive pathogens—specifically streptococcus species. A 5-day course of antibiotics may actually be as effective as 10 days’ worth [ 3 , 13 , 14 , 25 ]. To avoid recurrence, tissue maceration, edema, eczema, and venous insufficiency should be treated, though this may be difficult, especially in an ambulatory context [ 2 ].…”

“…Eine neue Metaanalyse auf der Basis von 43 Studien aus dem Jahr 2019 [6] kommt zu dem Schluss, dass es keine Evidenz dafür gibt, dass ein Antibiotikum in der Therapie überlegen ist und dass die intravenöse Applikation der oralen Gabe des Antibiotikums überlegen ist.…”

“…Detection of genetic tumour predisposition syndromes using electronic health records DOI: 10.1111/bjd.19215 DEAR EDITOR, The restricted tumour repertoire of certain genetic skin tumour syndromes, such as CYLD cutaneous syndrome (CCS), is well recognized. 1 Patients with CCS develop multiple benign cylindromas and spiradenomas, a presentation of skin appendageal tumours recognized to be exclusively associated with loss of CYLD function. 2 Consequently, pathology coding records, which capture tumourlevel data, can be interrogated to detect patients who may have undiagnosed CCS based on the presentation of multiple CCS-specific tumours over time.…”

“…DEAR EDITOR, There is no evidence to support using an antibiotic for longer than 5 days in cellulitis. 1 The National Institute for Health and Care Excellence recommends specialist review if cellulitis has not improved after 14 days despite antibiotics. 2 The 2017 Cellulitis Priority Setting Partnership prioritized research into diagnosis and treatment of cellulitis.…”
