Background Climacteric changes in women are associated with an increased probability of psychological symptoms. Identifying the relationship between adjustment to this period and mental health helps to plan for middle-aged women’s health improvement. Therefore, the present study aimed to investigate the relationship between climacteric adjustment (CA) and mental health in middle aged women. Method This cross-sectional study was conducted on 190 women aged 40 to 53 years. Mental health symptoms (including hypochondriasis, anxiety, depression, and social impairment) and CA were assessed using 28-item general health questionnaire and the CA questionnaire, respectively, as a self-report. Data were analyzed using linear and stepwise regression methods, and the fitting of the resulting conceptual model was assessed using AMOS software. Results The results showed that hypochondriasis score and social impairment, anxiety level and CA in the perfection dimension, and social impairment score and CA in perfection, decline in beauty, and sexual silence dimensions had an inverse relationship. Moreover, the relationship between anxiety score and CA in the reaction to end of menstruation and the relationship between social impairment and decline of femininity were positive and significant. Factor analysis of the conceptual model obtained from the study results demonstrated a good model fit (CMIN /DF = 0.807, P = .671). Conclusion The results showed a relationship between CA and psychological symptoms in middle-aged women. In other words, the level of hypochondriasis, anxiety, and social impairment symptoms decreased with increasing CA in sexual silence, perfection, and decline in beauty.

ZusammenfassungIn diesem Beitrag wird ein Forschungsprojekt vorgestellt, welches die Frage nach den Ursprüngen sexueller Schmerzen oder Schwierigkeiten beim Geschlechtsverkehr von Frauen untersucht und dabei eine Reihe theoretischer, methodologischer und empirischer Ansätze verfolgt. Hierzu wurden weltweite Prävalenzzahlen, die subjektive Perspektive von Frauen mit eigenen Erfahrungen mit Schmerzen oder Schwierigkeiten beim Geschlechtsverkehr auf mögliche Ursprünge sowie psychodynamische Überlegungen und Bezugssysteme anhand eines Literaturüberblicks, qualitativer Analysen (Grounded Theory, psychoanalytisch orientierte Forschung) und einer quantitativen Analyse (statistische Auswertung von Fragebogendaten) untersucht. Die Ergebnisse bieten diverse Anregungen für weitere Forschung und legen nahe, dass die durch die unterschiedlichen Perspektiven erarbeiteten Aspekte sowohl im Einzelnen tiefergehend untersucht werden sollten als auch im Hinblick auf ihre gegenseitige Beeinflussung und Verbundenheit. Darüber hinaus erscheint es für die klinische Praxis relevant, die verschiedenen Aspekte in die Behandlung einzubinden.

Background: There is low social awareness of vaginismus despite it being a relatively common female sexual dysfunction that makes vaginal penetration painful, difficult, and/or impossible. While existing literature on vaginismus has had a clinical focus on the affected genitalia, there is a lack of research on women’s help-seeking experiences of vaginismus from their perspective. Objectives: This integrative review’s objective was to explore: women’s help-seeking experiences of vaginismus, and how such help-seeking experiences impact their sense of self. Design: Cooper’s five-step integrative review approach was followed to develop a research question, a search strategy, selection criteria, and data evaluation, analysis, and presentation. Data Sources and Methods: A systematic search of the literature was completed in the following seven databases in January 2023: PsycINFO, ProQuest Central, PubMed, Scopus, CINAHL, Cochrane, and Embase. Out of the 373 articles found through database searches and additional citation searching, 22 studies were included in this review for meeting the eligibility criteria of having an empirical design, being written in English, and examining women’s help-seeking experiences for vaginismus and its impact on their sense of self. Results: Thematic analysis was used to summarize the findings from the included studies which were informed by 1671 participants. Help-Seeking Process, Medical Management, Help-Seeking and Sense of Self, and Holistic Care Recommendations from the Findings emerged as four major themes with corresponding subthemes. Conclusion: This review indicates that women continue to face difficulties in seeking and receiving help for vaginismus even through the healthcare system. However, the studies did not explicitly discuss how women’s help-seeking for vaginismus impacted their sense of self. This highlights an epistemological gap on how women’s help-seeking for their vaginismus impacts their sense of self, which can affect their treatment responses. Recommendations are provided for future healthcare and research to improve health outcomes for women with vaginismus.
