Aspirin for primary stroke prevention in elderly patients with vascular risk factors

Abstract: The effect of aspirin in primary stroke prevention is controversial in Western population, and no evidence is available in Asian population. We performed stroke subanalysis of the Japanese Primary Prevention Project (JPPP), which was a randomized controlled trial of aspirin vs no aspirin for primary prevention of vascular events in 14 464 patients aged over 60 years with hypertension, diabetes, and/or dyslipidemia. We evaluated the effects of aspirin on the risk of stroke and intracranial hemorrhage. Aspirin d… Show more

“…Cerebrovascular diseases are regarded as a group of major health problems for human beings worldwide, causing high morbidity, mortality as well as disability ( Qureshi, 2004 ). It is widely acknowledged that there are two major types of cerebrovascular diseases, including ischemic and hemorrhagic stroke, with ischemic stroke referred to as decreased supply to certain brain regions and hemorrhagic stroke as bleeding into or around the brain tissue ( Uchiyama, 2017 ; Gokcal et al, 2018 ).…”

“…In this study, the parent recommendation of “cautious” use of aspirin <325 mg for cardiac protection among older Africans >80 years was modified to “avoid” based on “lack of evidence of benefit versus the risk in individuals ≥80.” This recommendation was, however, rejected by the local experts. This experts' opinion contradicts a recent randomized controlled study among the Japanese older persons that showed that aspirin had no net benefit in stroke prevention in a 5‐year follow‐up . Another prospective population‐based study in the United Kingdom reported a sustained long‐term risk of disabling and fatal upper gastrointestinal bleeding with aspirin among older persons aged 75 years in a transient ischaemic attack …”

“…The microcirculation cannot be improved barely depending on aspirin and clopidogrel to carry out the antiplatelet treatment. The neuron will be damaged due to the factors of hypoxic-ischemic and the continuous effect of secondary oxidative stress [11,12]. Salviae miltiorrhizae and ligustrazine are traditional…”
