Background: Tinea is a superficial mycoses caused by pathogenic, keratin digesting fungi, called dermatophytes. In Mexico, the most frequent agent is Trichophyton rubrum, and on the other hand, Microsporum gypseum is the most important geophilic one, however this is infrequent. Objective: To determinate the frequency and prevalence of M. gypseum infections in a Dermatological Center of Mexico. Methods: Descriptive, retrospective, observational and transversal study of a 14 years (2002-2016) period. All the patient charts with tinea due to M. gypseum confirmed by culture were included. The following data were analyzed: gender, age, occupation, provenance, time of onset and clinical features. Descriptive statistics were used. Results: A total of 3,921 mycological studies were done with 64 isolations of M. gypseum (1.7% of the total). 21.88% were students and 20.32% were mature adults. The body regions involved were head/neck (34.3%), trunk (21.8%), forearms (15.6%) and thighs (10.9%). Study Limitations: Some limitations of our work were the study period and the number of negative cultures from the total performed at our micology laboratory. Conclusions: Microsporum gypseum has a wide world distribution however clinical cases due to this agent are scarce. This is one of the largest compilations of clinical cases due to this pathogen. Students and housewives were the main affected. It could be due to play on the ground, contact with pets, or domestic duties.

Introductionl'épidémiologie de ces affections surtout d'origine mycosique est peu connue en Côte d'Ivoire. Cette étude avait pour objectif de déterminer les différents aspects cliniques et fongiques des intertrigos.MéthodesD'avril à octobre 2012, une étude transversale a été conduite dans le service de dermatologie clinique du Centre Hospitalier et Universitaire de Yopougon (Abidjan-Côte d'Ivoire). Elle a concerné les patients vus en consultation et présentant des lésions des plis, évocatrices d'une atteinte mycosique. Les prélèvements de sérosités par écouvillonnage ou de squames par raclage à la lame de Bistouri ont été réalisés au niveau des lésions. Les agents fongiques responsables de ces lésions ont été identifiés après culture du produit biologique.RésultatsAu total, 200 patients ont présenté des lésions évocatrices d'intertrigo d'origine fongique. L'âge moyen des patients était de 29,8 ans (écart type = 11,1 ans). Les intertrigos mycosiques ont représenté 6,7 % des motifs de consultation. Une prédominance féminine a été observée (76,7%). Les lésions siégeaient majoritairement au niveau de l'aine (40,8%) et des plis inter-fessiers (36,9%). Les symptomatologies les plus observées étaient la macération (52,4%) suivie de la brûlure (18,4%). Les intertrigos étaient causés dans 89,3% des cas par les levures, parmi lesquelles Candida albicans (33%) et Candida parapsilosis (19,4%) étaient prédominants.ConclusionLes intertrigos d'origine mycosique affectent principalement les adultes jeunes de sexe féminin. Les lésions siègent préférentiellement au niveau des plis inguinaux et inter-fessiers. Les principaux agents étiologiques sont représentés par des levures du genre Candida.

Prevalence of dermatophytosis among Suse federal prison inmates using conventional identification methods and PCR-RFLP typing

Dermatophytosis is a common skin infection especially in closed area with high temperature and humidity. Prisons are considered as closed areas especially for long times existed inmates in addition to some risk factors that may increase dermatophytosis. To investigate the prevalence and the risk factors of dermatophytosis in Suse federal prison for men at Sulaimani, northern Iraq, a total of 1620 inmates were clinically screened. Clinically suspected dermatophytosis by specialists were 139 (8.5%), they distributed on 102 (73%) Tinea pedis, 18 (12%) Tinea cruris, 14 (10%) Tinea corporis, 4 (3%) Onychomycosis, and 1 (0.7%) tinea mannum. By microscopical, cultural and PCR-RFLP typing procedures 50 cases were confirmed that distributed on 25 (50%) tinea pedis, 15 (30%) tinea cruris, 9 (18%) tinea corporis and 1 (2%) onychomycosis. The causative agents were Trichophyton rubrum 16 (32%), Trichophyton mentagrophytes and Trichophyton interdigitale 14 (28%) for each, Epidermophyton floccosum 4 (8%) and Trichophyton schoenleinii 2 (4%). Trichophyton interdigitale was the most frequently isolated from tinea pedis (40%), Trichophyton rubrum and Trichophyton mentagrophytes from tinea corporis (33% for each) and Trichophyton rubrum from onychomycosis and tinea cruris (%100 and 40% respectively). Antifungal administration and frequency of bathing were with significant effect on identification of the causative agent by direct KOH test (P= 0.000 and P= 0.026 respectively) and on its recovery by culture method (P= 0.000 and P= 0.016 respectively). The non-significant risk factors were age (P= 0.07), education level (P= 0.82), smoking (P= 0.61), and duration of imprison (P= 0.557). KOH exam was fast and cheap but it is less specific and sensitive. Culture method of identification is essential to identify species of dermatophytes, but it was time consuming and with problems related with confusion in dissemination between species morphologically that could be avoided by PCR-RFLP method which we consider it as a golden method for identification in spite of its high cost but it is with less time, correct identification, and highest sensitivity.
