BackgroundThe novel coronavirus (SARS-CoV-2) pandemic spread rapidly worldwide increasing exponentially in Italy. To date, there is lack of studies describing clinical characteristics of the population most at risk of infection. Hence, we aimed to identify clinical predictors of SARS-CoV-2 infection risk and to develop and validate a score predicting SARS-CoV-2 infection risk comparing it with unspecific surrogates.MethodsRetrospective case/control study using administrative health-related database was carried out in Southern Italy (Campania region) among beneficiaries of Regional Health Service aged over than 30 years. For each subject with Covid-19 confirmed diagnosis (case), up to five controls were randomly matched for gender, age and municipality of residence. Odds ratios and 90% confidence intervals for associations between candidate predictors and risk of infection were estimated by means of conditional logistic regression. SARS-CoV-2 Infection Score (SIS), was developed by generating a total aggregate score obtained from assignment of a weight at each selected covariate using coefficients estimated from the model. Finally, the score was categorized by assigning increasing values from 1 to 4. SIS was validated by comparison with specific and unspecific predictors of SARS-CoV-2 infection.ResultsSubjects suffering from diabetes, anaemias, Parkinson’s disease, mental disorders, cardiovascular and inflammatory bowel and kidney diseases showed increased risk of SARS-CoV-2 infection. Similar estimates were recorded for men and women and younger and older than 65 years. Fifteen conditions significantly contributed to the SIS. As SIS value increases, risk progressively increases, being odds of SARS-CoV-2 infection among people with the highest SIS value (SIS=4), 1.74 times higher than those unaffected by any SIS contributing conditions (SIS=1).ConclusionThis study identified conditions and diseases making individuals more vulnerable to SARS-CoV-2 infection. Our results are a decision-maker support tool for identifying population most at risk allowing adoption of preventive measures to minimize a potential new relapse damage.

L´infection par le SARS-CoV-2 représente une grande source de préoccupation et une nouvelle menace pour les patients immunodéprimés. Les patients atteints de maladies inflammatoires chroniques de l´intestin (MICI) courent un risque accru d'infection, en particulier lorsqu'ils ont une maladie active et prennent un traitement immunosuppresseur. Ce travail vise à évaluer les caractéristiques cliniques, biologiques et radiologiques ainsi que la gestion et les résultats du COVID-19 chez trois malades présentant une MICI, recrutés sur une période de 3 mois au sein du Centre Hospitalier Universitaire Hassan II Fès, Maroc. Tous les malades étaient en poussée de la maladie. Tous avaient des symptômes légers ou étaient asymptomatiques. Aucun changement ou retard dans leurs schémas thérapeutiques n'est survenu, et aucun n'a développé un COVID-19 sévère. Les résultats du test de transcriptase inverse-réaction en chaîne par polymérase (RT-PCR) étaient positifs chez tous les patients. Radiologiquement, nous avons retrouvé dans un cas des opacités en verre dépoli sur le scanner thoracique. Le traitement était basé sur l'association d'hydroxychloroquine et d'azithromycine. L'évolution sous traitement était bonne pour tous les patients. Ce rapport préliminaire suggère que les malades présentant une MICI ne courent peut-être pas un risque plus élevé de développer le COVID-19 par rapport à la population générale.

La enfermedad inflamatoria intestinal (EII) abarca un grupo heterogéneo de patologías inflamatorias del tracto gastrointestinal que afectan de manera significativa las actividades laborales, educativas y sociales de las personas que la padecen. Debido a la innovación en el manejo farmacológico, el tratamiento quirúrgico se ha reducido en las últimas décadas; sin embargo, sigue teniendo un papel importante. Las indicaciones quirúrgicas se dividen en cirugía electiva (por falla o intolerancia al tratamiento médico, displasias, estenosis, cáncer, etc.) y de urgencia (colitis grave refractaria a tratamiento médico, megacolon tóxico, obstrucción intestinal, perforación, sangrado masivo, etc.). Existen múltiples opciones quirúrgicas dependiendo del tipo de afección, el sitio y la extensión, entre las que se pueden mencionar la proctocolectomía restaurativa con reservorio ileoanal, la proctocolectomía con ileostomía terminal, la proctocolectomía con ileostomía continente, la colectomía subtotal con anastomosis ileorrectal, la segmentectomía y la estricturoplastia, entre otras, cada una con sus indicaciones y complicaciones propias que se deben tener presentes al momento de elegir una opción. Este estudio tiene como objetivo realizar una revisión actualizada de las indicaciones y las complicaciones quirúrgicas en pacientes con EII.
