Arthritis associated with inflammatory bowel disease in children

Abstract: The records of 102 children with inflammatory bowel disease (44 with ulcerative colitis, 58 with Crohn's disease) were reviewed for evidence of joint disease unassociated with erythema nodosum. Thirteen children had arthritis, four had ulcerative colitis, and nine had Crohn's disease. Arthritis tended to be pauciarticular; ankles, knees, elbows and hips were most commonly affected. In three patients arthritis preceded bowel symptoms, in two the onset of arthritis and bowel disease was concurrent, and in eight … Show more

“…Perhaps this explains why the proportion of our subjects with enthesitis was similar to the background prevalence among otherwise healthy children [13]. The widespread use of these therapies, often in combination, for many children with IBD may also explain the lower rates of arthritis emerging from more recent studies [5,23] in comparison with older studies [2,3]. Of note, several potentially eligible subjects with more active bowel disease declined participation in this study.…”

“…As such we were underpowered to assess the relationship between enthesitis with other clinical characteristics, including arthritis, HLA-B27 positivity, and IBD phenotype and disease activity. Previous studies of children and adults with IBD, for example, have showed that arthritis tends to occur more commonly among those with colonic disease [3,21,22]. Future studies should be powered to make these statistical comparisons.…”

“…As previously mentioned, the arthritis is pauciarticular. Arthritis begins in the lower limb and the most frequently affected joints are knees, ankles, hips, and elbows, while the finger-joints are less commonly attacked [6,14-16].…”

“…There are several studies that characterize the MSK EIMs in adult IBD; however, data for p‐IBD are sparse. In 1986, Passo et al (10) were the first to describe the association of arthritis in patients with p‐IBD. In this retrospective study, arthritis was described in 9% of children with UC and in 15.5% of children with CD.…”
