3155Purpose-Cervical artery dissections (CDs) are among the most common causes of stroke in young and middle-aged adults.The aim of this scientific statement is to review the current state of evidence on the diagnosis and management of CDs and their statistical association with cervical manipulative therapy (CMT). In some forms of CMT, a high or low amplitude thrust is applied to the cervical spine by a healthcare professional. Methods-Members of the writing group were appointed by the American Heart Association Stroke Council's Scientific Statements Oversight Committee and the American Heart Association's Manuscript Oversight Committee. Members were assigned topics relevant to their areas of expertise and reviewed appropriate literature, references to published clinical and epidemiology studies, morbidity and mortality reports, clinical and public health guidelines, authoritative statements, personal files, and expert opinion to summarize existing evidence and to indicate gaps in current knowledge. Results-Patients with CD may present with unilateral headaches, posterior cervical pain, or cerebral or retinal ischemia (transient ischemic or strokes) attributable mainly to artery-artery embolism, CD cranial nerve palsies, oculosympathetic palsy, or pulsatile tinnitus. Diagnosis of CD depends on a thorough history, physical examination, and targeted ancillary investigations. Although the role of trivial trauma is debatable, mechanical forces can lead to intimal injuries of the vertebral arteries and internal carotid arteries and result in CD. Disability levels vary among CD patients with many having good outcomes, but serious neurological sequelae can occur. No evidence-based guidelines are currently available to endorse best management strategies for CDs. Antiplatelet and anticoagulant treatments are both used for prevention of local thrombus and secondary embolism. Case-control and other articles have suggested an epidemiologic association between CD, particularly vertebral artery dissection, and CMT. It is unclear whether this is due to lack of recognition of preexisting CD in these patients or due to trauma caused by CMT. Ultrasonography, computed tomographic angiography, and magnetic resonance imaging with magnetic resonance angiography are useful in the diagnosis of CD. Follow-up neuroimaging is preferentially done with noninvasive modalities, but we suggest that no single test should be seen as the gold standard.

Background: The right to prescribe drugs remains a contentious issue within the chiropractic profession. Nevertheless, drug prescription by manual therapy providers is currently an important topic. Notably, physiotherapists in the United Kingdom were recently granted limited independent prescribing rights. Reports suggest that physiotherapists in Australia now want those same rights, and as such a review of chiropractors' general attitudes toward drug prescription is needed. Objective: To examine the literature concerning chiropractors' attitudes toward drug prescription rights and to compare the opinions of chiropractors currently licensed to prescribe medication with those in the profession who are not. Methods: This was a narrative review, consisting of a formal literature search and summary of included articles. Electronic databases searched included the Cumulative Index to Nursing and Allied Health Literature, PubMed, and the Index to Chiropractic Literature. Inclusion criteria consisted of prospective studies published in English in peer-reviewed journals. Studies were required to contain data on chiropractors' opinions toward medication prescription rights. Results: Of 33 articles identified, a total of seven surveys were included in the review. Of these, there was a general split in opinion among chiropractors regarding the right to prescribe drugs in chiropractic practice. Those supportive of prescribing rights favoured a limited number of over-the-counter and/or prescription-based medications such as analgesics, anti-inflammatories, and muscle relaxants. When questioned on full prescribing rights, however, chiropractors were generally opposed. In jurisdictions where chiropractors are currently licensed to prescribe from a limited formulary, such as in Switzerland, the majority perceived this right as an advantage for the profession. Moreover, continuing education in pharmacology was viewed as a necessary component of this privilege. Conclusions: Based on the literature to date there is a general split in chiropractors' attitudes toward drug prescription rights. This split is most pronounced in countries where chiropractors are not licensed to prescribe medications. Notwithstanding, this is an important topic in chiropractic currently and warrants both further discussion and research to determine future directions and the implications of either pursuit or denial of prescription rights by chiropractors. Future surveys and/or qualitative studies of other chiropractors' opinions toward gaining prescription privileges would be timely.

STRESZCZENIEWstęp. W leczeniu skolioz idiopatycznych (SI), poza rekomendowanymi specyficznymi metodami fizjoterapeutycznymi, stosuje się także niespecyficzne formy terapii manualnej (NTM). Celem pracy była ocena skuteczności NTM (terapia manualna, chiropraktyka, osteopatia) stosowanych w leczeniu dzieci i młodzieży z SI.Materiał i metody. W badaniu wykorzystano analizę systematycznych przeglądów piśmiennictwa (analiza 1) oraz innych, aktualnych publikacji naukowych (analiza 2). Do analizy 1 włączono prace dotyczące wykorzystania NTM u osób z SI. Z analizy wyłączano prace dotyczące specyficznej fizjoterapii (SF), leczenia gorsetowego (G) oraz innych typów skolioz. Kryteria włączenia do analizy 2: terapia prowadzona z wykorzystaniem NTM; badani w wieku 10-18 lat z SI. Z analizy wyłączano prace: w których analizowano NTM w połączeniu z SF, G; dotyczące osób dorosłych; analizy pojedynczego przypadku; prace objęte analizą 1.Wyniki. Analiza 1 -w 6. przeglądach piśmiennictwa znaleziono 6 prac dotyczących oceny skuteczności NTM w leczeniu osób z SI. Wyniki tych prac są sprzeczne -od wykazujących redukcję kąta Cobba, do wskazujących na brak skuteczności leczenia. Analizowane prace charakteryzują się niską jakością metodologiczną -mała liczebność badanych, brak pełnego opisu grupy badanej, brak follow-up, brak grupy kontrolnej. Analiza 2 -łącznie znaleziono 217 prac. Żadna nie spełniała przyjętych kryteriów.Wnioski. 1. Do tej pory ukazało się niewiele prac weryfikujących skuteczność terapii manualnej, chiropraktyki oraz osteopatii w leczeniu osób ze skoliozą idio patyczną. 2. Większość badań to badania eksperymentalne o niskim poziomie metodologicznym lub obserwacyjne studia przypadku. 3. Nie można obecnie w sposób rzetelny ocenić sku teczności niespecyficznych zabiegów manualnych stosowanych w leczeniu chorych ze skoliozą idio patyczną. 4. Konieczne jest prowadzenie dalszych badań o od powiedniej jakości metodologicznej (badania pro spektywne, randomizowane, kontrolowane), aby w sposób właściwy ocenić przydatność niespecyficznych zabiegów manualnych w leczeniu chorych ze skoliozą idiopatyczną.Słowa kluczowe: terapia manualna, chiropraktyka, osteopatia, skolioza idiopatyczna, leczenie SUMMARY Background.
