Applicability of the Zwolle risk score for safe early discharge after primary percutaneous coronary intervention in ST-segment elevation myocardial infarction

Abstract: The ZRS appears to perform well in identifying low-risk STEMI patients who could be safely discharged within 72 hours of admission. Using the ZRS in our population could result in a more rational use of in-patient resources.

“…One Spanish study examined 276 patients with the Zwolle Risk Score, however, only did so retrospectively. 6 A prospective study from the same country looked only at low-risk patients, of which just 54 were targeted for early discharge, with no assessment of effect on cost. 12 The Safe-Depart Trial studied prospective implementation of the Zwolle Score in a Canadian hospital, but only 54 patients were enrolled, of whom just 27 had the risk score applied to their care.…”

“…The ZRS has been validated in several studies and can be used in up to 60% of all STEMI patients. 11,22,23 In this metaanalysis, the study by Melberg et al 13 and the EDAMI trial used the ZRS to identify low-risk STEMI patients (ZRS 3). The PAMI-II criteria designate as low-risk STEMI patients aged 70 years or less, with a left ventricular ejection fraction greater than 45%, one or twovessel disease, successful PCI and no persistent arrhythmias.…”

“…Показано, что данные факторы являются независимыми предикторами 30-дневной смертности. Пациенты с ≤3 баллами по шкале Zwolle имеют 0,5% риск 30-дневной смертности и могут быть безопасно выписаны из стационара в первые 72 ч. Валидность шкалы Zwolle была показана в целом ряде исследований [295][296][297][298].…”
