Applicability of the WHO Maternal Near Miss Criteria in a Low-Resource Setting

Abstract: BackgroundMaternal near misses are increasingly used to study quality of obstetric care. Inclusion criteria for the identification of near misses are diverse and studies not comparable. WHO developed universal near miss inclusion criteria in 2009 and these criteria have been validated in Brazil and Canada.ObjectivesTo validate and refine the WHO near miss criteria in a low-resource setting.MethodsA prospective cross-sectional study was performed in a rural referral hospital in Tanzania. From November 2009 unti… Show more

“…These determinants are known factors associated with maternal morbidity or mortality (12,16). Mean body mass index and maximum diastolic blood pressure were higher in the potentially life-threatening group, which may be explained by the fact that obesity is a risk factor for high blood pressure and (pre)eclampsia (16,17).…”

“…Importantly, case-fatality rates for the study populations in these countries were 3.2% (Brazil), 12% (Malawi) and 13% (Tanzania), which indicates that it is justified to state that all women with SAMM in these countries actually have "life-threatening" conditions. The low detection results were attributed to absence of sophisticated laboratory diagnostics and lack of manpower to perform extensive clinical monitoring in low-income countries (10,12). In the Netherlands, however, such laboratory diagnostics and human resources are available.…”

“…The threshold of five units of blood for identification of MNM according to WHO criteria led to underreporting of MNM in a low resource setting. 4 Five factor scoring system uses 3 units of threshold of blood with specificity of 93.9%. 5 The objective of this study was to evaluate the associated risk factors and determinants of near miss obstetric cases at B.P.…”
