Appendiceal mucocele: sonographic findings

Abstract: AM appears as an elongated echo-poor mass without posterior echo enhancement. The cyst wall is less distinct than what one would expect for a cyst. When encountering such a mass in the lower right abdomen, one should strongly suspect an AM. In such cases, appropriate diagnostic and therapeutic strategies are especially necessary to prevent rupture that results in development of pseudomyxoma peritonei.

“…Ultrasound is the firstline diagnostic modality for patients with acute abdominal pain or mass. Different sonographic findings of AM and AA have been described [3,16,17] . Appendix diameter 15 mm or more in USG examination has been determined as the threshold for AM diagnosis with a sensitivity of 83% and a specificity of 92% [3] .…”

“…1,2 In our case, the patient presented the typical US pattern characterized by a cystic lesion with the onion skin sign that is considered as a specific marker for appendiceal mucocele (Figure 1), 1,2 while the CT imaging showed a capsulated cystic mass with or without calcification of the wall ( Figure 2). 3,4 In our case the imaging was pathologically confirmed (Figure 3).…”

“…Ultrasound is the first line diagnostic modality for patients with acute abdominal pain or mass. Different sonographic findings of appendiceal mucocele and acute appendicitis have been described (16)(17)(18). Outer diameter of appendix 15mm or more in USG examination has been described as the threshold for the diagnosis of appendiceal mucocele with a sensitivity of 83% and a specificity of 92% (16), while for acute appendicitis it has been established as 6mm (19).…”
