Background: Fecal incontinence (FI) is a stressful condition for children and their parents that may affect the patient’s psychological well-being. Evaluating the patients’ psychological status may help physicians manage the disease effectively. Objectives: This study aimed to assess the emotional and behavioral disturbances in children with FI who were referred to the pediatric gastroenterology clinic in Mofid Children’s Hospital from April 2021 to 2022. Methods: This cross-sectional study included children (over four years old) with chronic constipation and fecal incontinence. The diagnosis of chronic constipation and FI were made according to Rome-IV criteria. The Child Behavior Checklist (CBCL) evaluated patients’ emotional, behavioral, and social problems. Results: One hundred one patients with a mean age of 7.96 years were enrolled in the study; 67.32% were males. According to CBCL, 12% (12 patients) indicated emotional and behavioral problems, with CBCL scores in the clinical or at-risk range. We detected anxious/depressed problems in five (4.95%), withdrawn/depressed problems in eight (7.92%), somatic complaints in seven (6.93%), social problems in eight (7.92%), thought problems in nine (8.91%), attention problems in seven (6.93%), rule-breaking behavior in two (1.98%), and aggressive behavior in nine (8.91%) patients. The risk of internalizing and externalizing disorders was reported in four (3.96%) and five (4.95%) patients. Also, eight (7.92%) and seven (6.93%) patients had clinical symptoms of internalizing and externalizing disorders, respectively. There was no significant relationship between patients’ age and gender with the CBCL scores in any subscales. However, there was a significant difference in the total score among the age groups (P = 0.04). Conclusions: The relatively high prevalence of emotional, behavioral, and social problems in our study corroborates the importance of psychological screening of children with FI during the treatment process.

Introduction constipation affects up to 29.6% of children worldwide, making it one of the most common gastrointestinal illnesses in the pediatric population. As a functional disorder, the development of constipation is mostly influenced by a child´s psychosocial condition, even considered as one of important contributing factors. This systematic review aims to evaluate the relationship between psychological stress with constipation in the pediatric population. Methods three online databases were searched as study sources, including PubMed, the Cochrane Library, and Google Scholar. Study selection was carried out using the PRISMA diagram. Studies that met the eligibility criteria were then included in the data extraction and synthesis. The study quality assessment was done using the Joanna Briggs Institute's (JBI) critical appraisal checklist. Results eleven studies are included in this systematic review, consisting of four cross sectional studies, four case control studies and three cohort studies. The included studies have good quality based on the assessment. Majority of the studies showed a significant relationship between psychological stress and constipation in children. Psychological stress in children can be classified into family-related stressors, school-related stressors, exposure to stressful life events, stress related to psychological disorders, and other factors. Conclusion psychological stress and burden are associated to constipation in children. To overcome functional constipation in children, a collaborative effort is required between parents, children, and the healthcare professional.

The Prevalence of Social Anxiety in Children with Chronic Functional Constipation

Background: Functional constipation (FC) is a common disease in children. This disease can cause many behavioral and mental problems and ultimately reduce the quality of life. Reciprocally, psychological factors can play a role in the development and persistence of FC. Objectives: In this study, we aimed to determine the prevalence of social anxiety in children with chronic FC in Arak city, Iran. Methods: In this cross-sectional study, 200 children between 10 - 17 years with chronic FC referred to Amirkabir Hospital in Arak, Iran, which is a specialized center, were enrolled. This study was done in 2022, and the participants' disease was diagnosed based on Rome ΙV criteria. The Social Anxiety Scale for Adolescents (SASA) was completed by children under the guidance and supervision of their parents. Statistical analyses were performed in SPSS 22 using the Chi-square test and independent sample T-test. Results: The participants’ mean age (SD) was 13.43 ± 1.54 years. The prevalence of social anxiety was estimated at 67.5% (95%CI: 60.6 - 73.4), of whom 70% had moderate anxiety, and 30% had severe anxiety. There was no significant difference in the prevalence of social anxiety between the age groups (P = 0.167). The prevalence of social anxiety was significantly higher in girls (74.8%) than in boys (59.8%) (P = 0.034). Also, girls showed a significantly higher prevalence of severe social anxiety than boys (P = 0.023). Conclusions: In this study, the prevalence of social anxiety was found to be considerably high in children with chronic FC. There was no significant difference between different age groups in terms of the prevalence of social anxiety; however, the prevalence and severity of this disease were significantly higher in girls than in boys.
