Antipsychotic medication prescribing trends in a tertiary care hospital

Background: Aim of the study was to evaluate the drug utilization pattern of antipsychotics in schizophrenia patients in a tertiary care hospital.Methods: A retrospective observational study was conducted over a period of one year. Demographic data and drug utilization pattern of antipsychotics was collected from the surveyed prescriptions. Using WHO prescribing indicators data was analysed. Results were expressed in percentage.Results: Out of 300 prescriptions analysed, males were 58% and females 42%. Majority of schizophrenia patients were in age group of 15 to 45 years 70.33%. Olanzapine 75% was most common antipsychotic drug prescribed followed by risperidone 10.7%. Haloperidol 9.6%, fluphenazine 3.7% and chlorpromazine 1% were the other antipsychotics prescribed. Atypical antipsychotics 85.7% were commonly prescribed than conventional ones 4.3%. Concomitant drugs prescribed were anticholinergics, antihistaminics, sedative-hypnotics, antidepressants, mood stabilizers, antiulcer drugs and vitamin B complexes. Average number of drugs prescribed per prescription 3.26.Conclusions: Nowadays atypical antipsychotics are preferred over conventional ones because of their less side effects. Drug utilization study helps in rational usage of drugs important for patient care.

Background: A mental disorder or psychiatry disorder is a behavioural or mental pattern that causes significant distress or impairment of personal functioning. Schizophrenia is one of the most common psychiatric disorders. Aim of the study is to study drug utilization pattern in schizophrenia patients in outpatient department of psychiatry.Methods: A prospective and observational study was conducted in psychiatry outpatient department between May 2018 to October 2018 at Government Medical College and Hospital Aurangabad. Approval from ethics committee was taken. Data collected was of age, sex, final diagnosis, drugs prescribed to patients from case report form. The data obtained was then analyzed in Microsoft excel.Results: Total prescriptions of 129 patients were analyzed. Out of 129 patients 99 (76.74%) were male, while 30 (23.26%) were female. The mean age of the patients admitted in Psychiatry OPD was 36.71±11.76 years. Number of drugs prescribed per patient being 3.40±1.12 (mean±SD). The most common drugs used were olanzapine (66.6%), trihexyphenidyl (51.9%), lorazepam (44.9%), risperidone (39.5%), trifluoperazine (27.9%).Conclusions: Our study shows that atypical antipsychotics were the most common drugs prescribed in patients. Olanzapine was the most common medication used followed by risperidone. Our prospective study is carried out to get a more comprehensive data so that we can improve the drug utilization in our hospital.

Drug utilization audit of antipsychotics using WHO methodology: recommendations for rational prescribing

Background: Utilization pattern of antipsychotics has undergone a major shift as the newly introduced atypical antipsychotics have heralded the market. Hence continuous studies on current prescribing trends are needed to provide most updated, effective and rational treatment of psychoses.Methods: This cross-sectional prospective study was conducted at Rohilkhand Medical College and Hospital, Bareilly, Uttar Pradesh, India. OPD tickets of the department of psychiatry for the year 2016-2017 were evaluated for a total of 310 cases. These prescriptions were used to determine the drug utilization of antipsychotics and for their appropriateness against the WHO recommended core prescribing indicators.Results: A total of 310 patients satisfied the inclusion criteria and a male predominance was seen (male:female= 1.3:1). The most common psychotic disorder diagnosed was schizophrenia (44%). Majority of the patients (76%) received monotherapy and the prescribing trend showed a higher use of atypical antipsychotics (89%) over typical ones (11%). The most commonly prescribed anti-psychotic drugs were olanzapine (31%), followed by risperidone (26%). On analysing prescriptions according to the WHO core prescribing indicators it was observed that average number of antipsychotics prescribed per prescription was 1.32. 24% of drugs were prescribed by generic name and 5.2% were administered via injectable route. Drugs prescribed from National list of Essential Medicine, 2015 constituted 46%.Conclusions: The high inclination towards the use of atypical antipsychotics observed in our study correlates with the global changing trends in the treatment of schizophrenia. WHO prescribing indicators should be adhered to, to impart rational prescribing.
