Antibiotics prior to embryo transfer in ART

Abstract: Bacteriological catheter analysis was performed on 284 of 350 women randomised. In the antibiotic arm 154/178 (86.5%) were analysed. In the control arm, 130/172 (75.6%) were analysed.

“…The use of antibiotics has been widely studied to treat BV and prevent preterm birth (reviewed in []). But the usefulness of antibiotic use before embryo transfer remains controversial; while it is efficient in reducing upper genital tract contamination, no beneficial role has been observed in pregnancy outcome . This can be due to the lack of specificity of broad‐spectrum antibiotics that could impair not only the growth of dysbiotic bacteria, but also the protective Lactobacilli .…”

“…A recent Cochrane review analyzed randomized controlled trials in the literature that investigated antibiotics at embryo transfer (36). Only four potential studies were identified, of which three were excluded.…”

“…Although administration of antibiotics reduced microbial contamination as defined by culture of embryo transfer catheter tips, the clinical pregnancy rate was 36% in those receiving antibiotics and 35.5% in those not receiving antibiotics (odds ratio 1.02, 95% confidence interval 0.66-1.58) (37). The reviewers concluded that more evidence is needed with live birth as the primary outcome (36).…”

“…Endometrial implantation is the single most important event determining the success of embryo transfer in assisted reproductive therapy (ART) (2). Other factors include the presence of microbial colonization of the upper genital tract (3) and possibly utero-cervical microbial colonization (4), which has been shown to be an independent and significant factor determining the success of assisted reproductive treatments (5).…”
