Diabetic food ulcer is one of the most prevalent long-term diabetic consequences, which has a significant financial and social impact on people, families, and the health system. It's also long-term consequence account for direct medical expenditures and lengthy periods of disability. Aim: This study aimed to evaluate the effect of integrating health belief model in a prophylactic diabetic foot care program on knowledge, beliefs, practices and risk for foot ulcer of diabetic elderly. Methodology: A quasi-experimental research design was utilized in this research. It was done at the medical outpatient clinics affiliated to Tanta University Hospitals. A purposeful sample of 300 older people with diabetes was selected. Two tools were utilized for data gathering. A structured interview schedule was the first tool with questions about the sociodemographics of senior patients, knowledge, reported practices, and perceived believes of diabetic elderly about foot ulcer. Second tool: A neurological foot and peripheral vascular examination was used as an assessment checklist to determine the risk of foot ulcers. Results showed a statistically significant improvement in elderly total knowledge (7% to 85.30%), reported practice (40.30% to 93.30%), total perceived beliefs (34.2455 to 44.9000) correspondingly pretest compared posttest. The degree of knowledge, reported practice, perceived believes, and foot ulcer risk scale of older diabetic patients improved after implementing the preventative diabetic foot care program. Conclusion and recommendation: there was a positive correlation between knowledge, reported practices, neurological and peripheral vascular risk exposure post applying program. Application of further research on large samples and other settings for generalization.

Peripheral artery disease is a frequent cardiovascular ailment in which constricted arteries restrict blood flow to our limbs. When we have Peripheral Artery Disease in our limbs, our legs haven't had enough flow of blood to increase production. The capillaries that nourish the lower legs narrow and stiffen in peripheral vascular disease, making walking harder. Reduced blood flow can cause cartilage and necrosis in the limbs. Peripheral arterial disease (PAD) is usually caused by multilevel atherosclerotic disease, typically in patients with a history of cigarette smoking, diabetes mellitus, or both. Intermittent claudication (IC), an early manifestation of PAD, commonly leads to reduced quality of life for patients who are limited in their ambulation. Percutaneous intervention for peripheral artery disease has evolved from balloon angioplasty for single lesions. To achieve good long-term efficacy, a close follow-up including objective tests of both the arterial lesion and hemodynamic status, surveillance of secondary preventive measures, and risk factor control. A 65-year-old man was admitted to the hospital with the following symptoms: worsening left severe calf pain, fatigue, cramping, aching, pain in the legs or arms that worsens when walking or using the arms, tightness in the muscles of the legs that worsens with exercise and improves with rest. He has a diabetes mellitus history. The lower extremity pulse was diminished or absent on physical examination, but all other vital signs were typical. Pulses in the left dorsalispedis and posterior tibialis could not be felt. The results of the cardiac examination were average. The patient has numbness in one leg, pain in both legs, decreased energy in both upper and lower limbs, ulceration, gangrene in the lower extremities, Hair loss, weakness, and dependent rubor (redness caused by swelling) present. He had audible bruits, a nonhealing lower extremity wound, and a delayed capillary refill, so he started treatment as soon as possible. The primary focus of this study is on professional management and outstanding nursing care, which may provide the holistic care that peripheral arterial disease necessitates while also effectively managing the challenging case.

Background: Diabetic foot ulcer is a serious complication of diabetes and a major cause of morbidity and mortality among diabetic elderly patients, and good prevention guideline programs will inhibit the diabetic foot ulcer and further complication. Aims: This study aimed to evaluate the effect of a foot ulcer prevention guideline program for elderly diabetic patients at EL- Fayoum General Hospital. Research design: A quasi-experimental research design was used to utilize this study. Setting: The study was conducted at the medicine outpatient clinic in EL- Fayoum General Hospital. Subjects: A purposive sample was used to choose 300 elderly diagnosed with diabetes. Tools for data collection: Two tools for data collection were used. 1st tool: Structured interviewing questionnaire included demographic characteristics of elderly patients, past and current medical history, knowledge assessment questionnaire, patients reported practice questionnaire. 2nd tool: Observational checklist to assess foot ulcer risk included neurological foot assessment and peripheral vascular assessment. Results: It was revealed that there was a highly statistically significant improvement in total knowledge, good score and satisfactory reported practice represented as (7% to 85.3%) and (40.3% to 93.30%) respectively pretest versus posttest. In addition, there was a highly statistically significant improvement in total neurological foot items and total right and left peripheral vascular items represented as (14.23±3.004 to 16.97±1.800), (11.38±2.57 to 12.22±2.11) and (11.32±2.63 to 12.15±2.14) respectively pretest versus posttest. Conclusion: The current study concluded that after applying the foot ulcer prevention guideline program, level of knowledge, reported practice and foot ulcer risk scale of elderly diabetic patients improved. Recommendation: Periodic prevention guideline program and reduction sessions regarding the prevention of foot ulcer in diabetic elderly patients.
