Background and Purpose. There is a wide variation in the anatomy of the CW in different individuals and population groups. The purpose of this study was to determine the proportion of variant anatomy of the circle of Willis (CW) and associated anomalies in patients with suspected cerebrovascular disease referred for cerebral computed tomography angiography (CTA) in two tertiary hospitals in Kenya. Methodology. This was a cross-sectional descriptive study conducted on 94 patients referred for cerebral CTA at the Kenyatta and Nairobi hospitals from August 2017 to February 2018. MIP and 3D reformatted images were analyzed by two senior radiologists to determine the final configuration of the CW and presence of vascular pathology. Vessels with diameters <0.8 mm were considered to be absent or hypoplastic. Chen et al. classification was used to determine the final configuration of CW. Results. Complete CW was seen in 37.2% (37.7% vs. 36.6% in males and females, respectively, p=0.909). Type A variant was the commonest anterior variant at 78.7%. Type E variant was the dominant posterior variant at 41.5%. Fetal PCA was demonstrated in 25.5%, unilateral > bilateral fetal PCA. Aneurysms were seen in 24.5% of patients. ACoA aneurysms were commonest at 43.6%. AVMs were seen in 8.5% of patients. Azygous ACA, fenestration, and duplication of vessels and persistent TA were not demonstrated. There is no significant association between aneurysms/AVMs and CW configuration. Conclusion. The findings in this study demonstrate slight differences in the CW configuration. A higher proportion of complete anterior CW was seen in female patients. Type A anterior and type E posterior circulation variants were the commonest in both males and females. No significant association was demonstrated between CW configuration and occurrence of aneurysms/AVMs in this study. Various methods of classification of CW configuration have been proposed with no standardized method to date, thus the need for consensus building between neuroanatomists, neurologists, neurosurgeons, and neuroradiologists for ease of comparison between similar future studies on the CW.

Configuration of the circle of Willis and its two parts among Egyptian: a magnetic resonance angiographic study

Background: We hypothesized that the collateral circulation differs in different ethnic groups. So, the aim of our work was to study variations of the circle of Willis (COW) among Egyptian and to compare our findings with the findings of other nationalities. Materials and methods: One hundred patients were studied using magnetic resonance angiography (3D-TOF-MRA). Frequency and morphologic variations in COW were studied. The diameters of the arteries of the anterior and posterior circle were verified. Finally, the differences among the mean diameters of these arteries regarding age and sex were also studied. Results: Complete, partially complete and incomplete COW were encountered in 28%, 38% and 34% in the studied cases. The incomplete anterior circle was found in 34% (10% isolated incomplete anterior circle and 24% combined incomplete anterior and posterior circles) and the incomplete posterior circle came across in 62% (38% isolated incomplete posterior circle and 24% combined incomplete anterior and posterior circles). Seven anterior circle variations were found. The commonest type was the classical type "a" with a prevalence of 56%, being higher in male (57.1%). The 2 nd common type was type "g" (hypoplasia or aplasia of the anterior communicating artery) with a prevalence of 24%, being higher in male (66.7%). Six posterior circle variations were found. The commonest variation was the classic type "a" with a prevalence of 26%, being higher in male (61.5%). Posterior circles types "d, e, h" (18%, 24%, 20%) constituted 62% and were characterised by hypoplasia/absent of the posterior communicating arteries. Conclusions: The prevalence of complete COW (classic or textbook type) was encountered only 28% of the studied cases. Variations of COW were found to be more common in the posterior circulation (62%). The incomplete anterior circle was found in 34% and it is mostly caused by hypoplasia or aplasia of the anterior communicating artery which was found to be more common compared to the literature.

This study aimed to compare the corrosion cast models of the porcine head arterial system with three-dimensional (3D) reconstructions using cone beam computed tomography (CBCT) of these cast models. Six heads from sows were simultaneously injected through both carotid arteries with Duracryl Plus for corrosion cast technique and an additional head, also from another one sow head, was filled with saturated lead tetroxide (Pb3O4) in a 10% hot water solution (40°C) of gelatin for CBCT study. Two-dimensional (2D) images were stored in Digital Imaging and Communications in Medicine (DICOM). Subsequently, segmentation and postprocessing of these images were performed by using various software programs. The 3D models were found to be compatible with the corrosion cast models. It was observed that osseous structures and arteries were clearly identified on CBCT images. Specimen scan, segmentation, and post segmentation had a duration of 10-15 minutes, 4 hours, and 15 minutes, respectively. The internal carotid artery, external carotid artery, and its main branches were seen well on 3D models. In conclusion, it is considered that 3D models and images can be effectively used in anatomy education, radiological evaluations, pathological and variational investigations.
