Background: The aim of the study was to assess results of interventions for iliocaval obstruction or compression in patient with acute and chronic venous disease. Methods: Patients with chronic venous insufficiency (CVI) C3-C6 (Clinical, Etiology, Anatomy, Pathology -classification of venous insufficiency) and acute deep venous thrombosis were assessed by ultrasound scan, computed tomography, venography, and/or intravascular ultrasound, and if an obstruction in their iliocaval or iliofemoral segments were confirmed, they underwent venoplasty and stenting. Acute deep venous thrombosis cases were treated with pharmacomechanical and/or catheter-directed thrombolysis and residual obstruction was then stented. Results: One hundred eighteen consecutive limbs were treated between October 2011 and December 2017. Thirty-two limbs had an active ulcer (27%), 27 limbs had healed ulcer or advanced skin changes (23%), 39 limbs had swelling with or without other symptoms of CVI (33%), 15 limbs had acute symptomatic deep venous thrombosis (13%), and the residual 5 iliac vein cases were causing pelvic congestion syndrome (4%). Patency rates of the stents in acute cases were 84.6%, 76.9%, 76.9%, and in chronic cases (combined thrombotic and nonthrombotic) 93.1%, 91%, and 89.9% in 3, 6, and 12 months, respectively. A relief of symptoms was achieved in 81.5 % of limbs at some stage during the first 12 months (most of them within the first 3 months), although at the end of this period only 59.3% remained free of symptoms. There was no limb loss and no mortality within 30 days from the intervention. Conclusions: Iliocaval and iliofemoral venoplasty and stenting in both acute and chronic obstruction cases can be performed safely with good patency rates and reasonable improvement of symptoms of CVI.

Cardiac output during exercise increases by as much as fivefold in the untrained man, and by as much as eightfold in the elite athlete. Increasing venous return is a critical but much overlooked component of the physiological response to exercise. Cardiac disorders such as constrictive pericarditis, restrictive cardiomyopathy and pulmonary hypertension are recognised to impair preload and cause exercise limitation; however, the effects of peripheral venous obstruction on cardiac function have not been well described. This manuscript will discuss how obstruction of the iliocaval venous outflow can lead to impairment in exercise tolerance, how such obstructions may be diagnosed, the potential implications of chronic obstructions on sympathetic nervous system activation, and relevance of venous compression syndromes in heart failure with preserved ejection fraction. Preload Reserve and Venous ReturnPreload reserve describes the dependence of cardiac output on venous return to the heart, independent of heart rate and contractility, and is a critical feature of how the body responds to changes in posture and exercise. Increased heart rate, vasoconstriction, and cardiac inotropy are well-described features of the baroreceptor-mediated responses to standing and exercise; however, the principal mechanism for increasing stroke volume is best explained by the ventricular function curves

ZusammenfassungBenigne zentralvenöse thorakale Obstruktionen haben bei Hämodialysepatienten eine hohe Prävalenz. Die symptomatischen Stenosen führen zur Verschlechterung der Lebensqualität sowie zu Dysfunktionen der arteriovenösen Dialysezugänge bis hin zum Verschluss. Auch eine Implantation eines zentralvenösen Katheters kann dadurch erschwert oder unmöglich gemacht werden. Zu den Risikofaktoren zählen in erster Linie ein Vorhandensein oder eine Vorgeschichte eines zentralvenösen Katheters oder einer Schrittmachersonde. Die Diagnose kann meistens aufgrund der typischen Symptomatik vermutet werden, die digitale Subtraktionsangiografie bleibt als bildgebende Methode der Goldstandard der Diagnostik. Asymptomatische Patienten profitieren nicht von einer prophylaktischen Behandlung, diese kann oft zur raschen morphologischen und klinischen Progredienz der Stenose führen. Endovaskuläre Techniken stellen die primäre Therapieoption dar, die chirurgischen Maßnahmen sind Patienten ohne endovaskuläre Therapieoptionen vorbehalten. Die Standardtherapie ist die repetitive perkutane Ballonangioplastie. Eine Insertion eines nicht ummantelten Metallstents kann bei einem elastischen Recoiling bzw. bei einem Frührezidiv sinnvoll sein. Ein Einsatz eines Stentgrafts sollte in selektierten Fällen abgewogen werden. Der Stellenwert der neuen Techniken, wie die Anwendung dedizierter venöser Stents bzw. medikamentenbeschichteter Ballons, sollte im Rahmen von randomisierten Studien geprüft werden.
