An Interdisciplinary Clinical Approach for Workplace Violence Prevention and Injury Reduction in the General Hospital Setting: S.A.F.E. Response

Abstract: A reduction in nursing staff assault incidence rates was notable. Clinicians equipped with knowledge, skills, and resources can identify and defuse unsafe situations to prevent violence. This clinical approach shifts the focus from crisis intervention to crisis prevention, which reduces injury.

“…Although not ideal, at times hospital security guards may be left alone with disruptive, threatening patients. To manage and deescalate these situations they describe the need to have welldeveloped communication skills (Hills and Joyce, 2013;Lakatos et al, 2018;Morphet et al, 2018); however, they report that communication is not a focus of the training that they receive. They report that the training adequately prepares them to use restraint techniques, however One of the repeated concerns that hospital security guards raised was the risk of infection, particularly from aggressors who spit at them.…”

“…Improving the MDT understanding of the role of the hospital security guard in code black situations (Gillespie et al, 2012) and creating team building activities that include hospital security guards is needed, and has the potential to improve clinical leadership in these situations, which will improve patient outcomes, as well as maintaining hospital security guard well-being. Further, ensuring that there is adequate opportunity for hospital security guards to debrief, formally and informally, is known to protect well-being (Lakatos et al, 2018;Morphet et al, 2018). Defusing and debriefing (Sandhu et al, 2014) as part of the MDT, in a hospital security guards only forum, one-to-one with a peer or their manager, via the employee assistance program, or in any other way that hospital security guards require, is needed and should be available.…”

“…The preventive measures used to combat occupational violence that have been evaluated include educational training conducted with the hospital nursing team, which was shown to decrease the incidence rates of aggression. 16 Another study evaluated an electronic program addressing an algorithm of the waiting time of patients at a university hospital and noted a decrease in the incidence rate of violence among the health team. 17 Systematic reviews 18 19 have also sought to answer the question of the effectiveness of interventions carried out to prevent violence against health workers; however, authors 18 19 have found low to moderate evidence regarding prevention programs carried out in emergency departments.…”
