An Immunocompromised Child with Bloodstream Infection Caused by Two Escherichia coli Strains, One Harboring NDM-5 and the Other Harboring OXA-48-Like Carbapenemase

Abstract: We describe a 16-year-old neutropenic patient from the Middle East with bloodstream infection caused by two carbapenemaseproducing Escherichia coli isolates that we characterized by whole-genome sequencing. While one displayed meropenem resistance and was bla NDM positive, the other demonstrated meropenem susceptibility yet harbored bla OXA181 (which encodes a bla OXA48 -like enzyme). This report highlights the challenge of laboratory detection of bla OXA48 -like enzymes and the clinical implications of genoty… Show more

“…Mainly found in Klebsiella species, reports on the detection of bla oxa -carrying E. coli have increased in the last 3 years in different parts of the world, being reported in studies in Myanmar [85], the United States [86] and Thailand [87]. In all three studies, the isolated strains were co-expressing bla OXA-48 or its variants and bla NDM5 .…”

“…In the United States, a K. pneumoniae isolate carrying bla NDM-1 and bla OXA-232 was reported in another patient transferred from India (31). However, bla NDM-5 and bla have been reported only once before in the same patient but in different E. coli isolates (32). In summary we describe the occurrence of two clinically important carbapenemases (the "evil twins"), bla OXA-181 (chromosomally encoded) and bla NDM-5 (plasmid encoded), in a carbapenem-resistant K. pneumoniae clinical isolate in the United States.…”

“…However, there are no clinically available beta-lactamase inhibitors that inactivate MBLs. Published studies report successful use of combination ceftazidime-avibactam and aztreonam to treat patients infected with MBLs [7] , [8] , [9] , [10] , [11] , [12] . In vitro data suggests the combination is stable in the presence of the enzymes as aztreonam is not hydrolyzed by MBLs and avibactam will inhibit most aztreonam-hydrolyzing betalactamases.…”
