An evaluation of the predictive validity and inter‐rater reliability of clinical diagnostic criteria for senile dementia of Lewy body type

Abstract: Several recent autopsy studies suggest that senile dementia of Lewy body type (SDLT) may be the second most common neuropathologic cause of dementia in the elderly, accounting for 7 to 30% of all cases. Operational criteria for the antemortem clinical diagnosis of SDLT have already been proposed by our group. The performance of these is now examined by randomizing the case notes from a new series of SDLT, Alzheimer, and multi-infarct dementia patients for psychiatric assessment by four raters of varying clinic… Show more

“…We anticipate the clinical diagnosis to be accurate in an estimated 90 % of our cases, based on studies of the sensitivity and specificity of similar standardized diagnostic criteria used previously (Boller et al 1989 ;McKeith et al 1994). This is supported by our clinical diagnoses, which were correct in six of the seven cases for which we had neuropathological data.…”

“…However, many prospective clinicopathological studies have since shown that AD is the most common cause of dementia in the elderly (8). Based on recent clinicopathological studies, dementia with Lewy bodies (DLB) has emerged as the second most common degenerative pathology associated with dementia, accounting for 10 -25% of all cases (9)(10)(11)(12). Vascular pathology now appears to be the third most common cause of dementia, accounting for roughly one-fifth of the number of dementia cases (13,14).…”

“…There is a potential overlap between the two disorders. 26 Lewy body dementia is among those conditions persons with nonamnestic MCI might progress to. 2 Consequently, the prevalence of nonamnestic MCI might have been slightly underestimated in this study.…”
