Hajj health examination for pilgrims with asthma in Malaysia: An ethnographic study

Background Asthma was one of the top causes of hospitalization and unscheduled medical attendances due to acute exacerbations and its complications. In Malaysia, all pilgrims must undergo a mandatory health examination and certified fit to perform pilgrimage. We studied the current organisational and clinical routines of Hajj health examination in Malaysia with a focus on the delivery of care for pilgrims with asthma. Methods We conducted non-participant observation to obtain ethnographic understanding of Hajj health examination activities for 2019. Observations were guided by a checklist and recorded as notes that were analysed thematically. The study was conducted at 11 public (from each region in Malaysia, namely, North, South, East, West of Peninsular Malaysia, and Sabah and Sarawak of East Malaysia) and two private primary care clinics. Results We observed considerable variation in the implementation and practice of Hajj health examinations among the 11 public clinics but no marked variation among the private clinics. The short time span of between three to four months was inadequate for disease control measures and had put pressure on health care providers. They mostly viewed the Hajj health examination as merely a certification of fitness to perform the pilgrimage, though respiratory health assessment was often inadequate. The opportunity to optimise the health of pilgrims with asthma by providing the appropriate medications, asthma action plan and asthma education including the preventive measures was disregarded. The preliminary health screening, which aimed to optimise pilgrims’ health before the actual Hajj health examination was not appreciated by either pilgrims or health care providers. Conclusions There is great potential to reform the current system of Hajj health certification in order to optimise its potential benefits for pilgrims with asthma. A systematic approach to restructuring the delivery of Hajj health examination could address the time constraints, clinical competency of primary health care providers and resources limitations.

<p class="IOPAbsText">The number of pilgrims of various ages and levels of education experience many health problems such as hypertension, respiratory infections and heart problems. From these problems encourage researchers to determine the response of prospective pilgrims to of health services in Pademawu Pamekasan. The object of this study is the pilgrims in Pademawu Pamekasan Regency. The purpose of this study was to determine the response of the pilgrims in Pademawu District, Pamekasan Regency. This research uses a quantitative approach by using questionnaire instrument for data collection. The sampling technique uses a total sampling population count of 70 pilgrims. The study was conducted in the District of Pademawu Pamekasan to the Hajj pilgrims in 2018. The results of the study of the pilgrims' response to the quality of service include the response of the pilgrims, health services cognitive, affective, conative, age and education levels. Overall, the response of the haj pilgrims to the quality of service on the cognitive aspect was 29.5%. In the affective aspect of 4.69%. In the conative aspect 2.05%. At the age factor of 4.51% and at the aspect of Education level as much as 2.19%. The quality of health services affects the response of the pilgrims to the cognitive, affective and conative aspects but is not influenced by the age and level of education of the pilgrims.</p><div><p class="IOPKwd">Keywords: health response, hajj pilgrims, health services</p></div>

The ability of the Muslim community in Indonesia to perform the hajj pilgrimage, which fulfills Islam’s fifth pillar, is increasing. With the rise in life expectancy, the number of Indonesian elderly hajj pilgrims (EHP) is consequently growing. However, chronic conditions such as impairments, illnesses, and diseases cause an increase in morbidity and mortality among EHP. Thus, an innovative training program with practical tips for a healthy hajj experience is needed to improve the healthcare service to the EHP. This study aims to investigate the effects of an innovative training on the performance of Hajj Healthcare Workers of Indonesia (HHWI) and on the clinical outcomes of EHP. It was a quasi-experimental study. The population was all HHWI from the Surakarta embarkation in 2014. Meanwhile, the sample of this study was all 21 HHWI from Yogyakarta who received innovative training consisting of EHP special care training (theory and practice) in addition to basic national training. Twenty-one HHWI from Surakarta were randomly chosen as a control group and only received basic national training. Pre and post tests were used to assess knowledge of HHWI after training. The average pretest scores of the intervention and control groups were 48.50 and 48.07, respectively (p= 0.337), while the posttest score of the intervention group was 86.83 (p = 0.033). The measured parameters were the performance of HHWI, which is reflected by the EHP mortality rate, the number of outpatients, and the number of patients referred to the outpatients. We included the EHP under the supervision of HHWI, which has acquired training. EHP morbidity was evaluated as the number of outpatients and referred patients. There were 2216 outpatients from the intervention group as compared with 2144 in the control group, with most of them being 60-70 years old (p= 0,075). The number of referred EHP in the intervention and control groups was 10 and 30 patients, respectively (OR 2.94; p= 0.002). As for the mortality cases, we observed 7 EPH deaths during the hajj period (2 EPH from the intervention group vs 5 EPH from the control group; OR 0.263; p=0.207).
