“…Initial features vary, e.g., PMR without an elevated erythrocyte sedimentation rate (ESR) (3), and common mimicking conditions can present with the polymyalgic syndrome (4). There is no gold standard test and several diagnostic criteria have been used (5)(6)(7)(8). This may explain the lack of agreement in PMR diagnoses between primary care physicians and rheumatologists (9 -11) and the revision of diagnoses in 5-23% of patients with longterm followup (12)(13)(14).…”

“…1 Elevated ESR is also common in infections, can serve as a useful aid in the diagnosis of various clinical conditions, and has been shown to correlate with an unfavorable prognosis in neoplastic diseases and coronary artery disease. [2][3][4][5] In addition, serial ESR measurements allow monitoring the efficacy of therapeutic interventions ͑e.g., in rheumatoid arthritis͒. Although ESR is a commonly ordered laboratory test, it has several limitations: ͑1͒ slow turnaround time since the measurement alone takes 60 min to perform; ͑2͒ it is an open tube system and thus has biohazard risks; and ͑3͒ inaccurate results are possible due to nonvertical tube positioning or inaccurate/delayed reading.…”

“…However, none of the study patients was later diagnosed with another condition. Even the best criteria for PMR (12), including the Bird et al criteria (13) or Chuang et al criteria (14), lack sensitivity and specificity. There is no consensus about diagnoses that must be ruled out or investigations that must be performed before diagnosing PMR.…”
