Abstract. Fifty consecutive spinal injury patients who underwent simultaneous EMG of the peri-urethral and perianal striated muscles along with cystometrogram on a multiple channel recorder are analysed. Bladder filling, voiding and also influence of spasticity on the EMG activity of both perianal and periurethral striated muscles were compared. Periurethral striated EMG along with CMG has a better diagnostic value (95 per cent) as compared to perianal EMG-CMG which was diagnostic in 72 per cent patients.

ZusammenfassungHintergrund und Ziel: Das Elektromyogramm des M. sphincter ani externus wird in der Proktologie meist mit Oberflächenelektroden abgeleitet. Die Ableitung mittels konzentrischer Nadelelektrode ist nur wenig verbreitet, obwohl nur sie eine quantifizierende Beurteilung der Schädigung des äußeren Analsphinkters zulässt. Hierfür muss man Einzelpotenziale ableiten und auswerten. Patienten und Methodik: In der Literatur sind bisher nur wenige Erfahrungswerte der Dauer und Amplitude von Potenzialen motorischer Einheiten bekannt. Wir werteten normwertige Elektromyogramme von 29 Frauen und 31 Männern aus. Ergebnisse: Unsere Werte für die Potenzialdauer und Amplitudenhöhe betragen 4,53 ms und 0,71 mV für das Gesamtkollektiv. 95% der Potenziale liegen zwischen 3,11 ms und 5,95 ms bzw. zwischen 0,25 mV und 1,15 mV. Nur wenige Potenziale waren länger als 8 ms, keines länger als 10 ms. Nur weniger als 1% der Potenziale lag über 1,5 mV, kein Potenzial erreichte 3 mV. Die Polyphasierate lag bei etwa 2%. Schlussfolgerung: Die Ergebnisse zeigen, dass sich die Potenziale deutlich von denen anderer Muskeln unterscheiden. Wir hoffen, mit unseren Ergebnissen zur Verbesserung der klinischen Anwendbarkeit des Elektromyogramms des M. sphincter ani externus beigetragen zu können. Since 1991 we have known that the entire abdominal musculature has a distinct resting tone. All other muscles lose their tension almost completely during the resting phase. Only the craniopharyngeal and anorectal sphincter systems retain natural spontaneous activity, which ceases only when a hollow organ is opened. This permanent natural spontaneous activity is extremely sensitive. For example, it increases during coughing and speech. With combined positron emission and computed tomography, it is possible to observe and measure this natural spontaneous activity in the adjacent musculature very clearly. This fact is important for understanding and therapy of the function and failure of sphincter systems.
