Diabetes is highly prevalent throughout the world and imposes a high economic cost on countries at all income levels. Foot ulceration is one devastating consequence of diabetes, which can lead to amputation and mortality. Clinical assessment of diabetic foot ulcer (DFU) is currently subjective and limited, impeding effective diagnosis, treatment and prevention. Studies have shown that pressure and shear stress at the plantar surface of the foot plays an important role in the development of DFUs. Quantification of these could provide an improved means of assessment of the risk of developing DFUs. However, commerciallyavailable sensing technology can only measure plantar pressures, neglecting shear stresses and thus limiting their clinical utility. Research into new sensor systems which can measure both plantar pressure and shear stresses are thus critical. Our aim in this paper is to provide the reader with an overview of recent advances in plantar pressure and stress sensing and offer insights into future needs in this critical area of healthcare. Firstly, we use current clinical understanding as the basis to define requirements for wearable sensor systems capable of assessing DFU. Secondly, we review the fundamental sensing technologies employed in this field and investigate the capabilities of the resultant wearable systems, including both commercial and research-grade equipment. Finally, we discuss research trends, ongoing challenges and future opportunities for improved sensing technologies to monitor plantar loading in the diabetic foot.

In today's life, biomedical field reach at higher level because of highest development in portable devices and the equipment which are being helping the patients in monitoring and controlling different disease at different stages. But in many work environments, the worker has lot pressure and continuous movement on daily basis which tends to foot related diseases as getting exact details about the foot movement throughout the daily work routine is not possible. In available treatment options in such conditions, they are recommended to employ the compression therapy delivered by bandage, medical-grade stockings, or pneumatic compression devices. As a result of this the patients may suffer from painful period which may cause amputation also especially in case of sensory neuropathy.While these forms of therapy can produce the slight improvements, still it is costly solution and patient remain an issues related to rehabilitation. As alternate of this need the wearable, wireless, portable health monitoring system have recently introduce as a low-cost solution for supervision of foot health condition monitoring. The proposed system is to design and implementation of wearable device for foot for diabetic person for disease monitoring where the system will be capable to detect the level of foot ulcers related issues along with daily exercise level monitoring.

Diabetic foot ulcer (DFU) is one of the major concern of diabetes and it is rapidly increasing, in worst case scenario this may lead to amputation. The DFU can be avoided by the early detection and proper diagnosis. Many of the studies carried out highlights that, thermography is the most useful technique to measure the changes in the temperature of plantar surface and alerts to indicate the risk associated with DFU. The distribution of temperature does not have a fixed pattern across the patients, hence it makes the difficulty in measuring the appropriate changes. This gap will provide a scope to improve the analysis technique so as to measure the plantar surface temperature effectively and identify any abnormal changes. In this paper, the segmentation algorithm namely adaptive C means (ACM) for the image segmentation is discussed. ACM is based on the spatial information and this method includes the two stage implementation. In the first stage, nonlocal spatial information is added and in the second stage, spatial shape information is used in order to refine the constraint of local spatial. Outcome of the proposed method shows that ACM is very much effective and it outperforms the other existing methods.
