Amoxicillin-induced exanthema in young adults with infectious mononucleosis: demonstration of drug-specific lymphocyte reactivity

Abstract: These data suggest that real sensitization to amoxicillin and ampicillin may occur within infectious mononucleosis and may be detected in vivo and in vitro by means of skin tests and the LTT.

“…In one study it was reported that 90.7% of IM patients were given antibiotics before hospital admission or serology tests [12]. The incidence of skin rash is higher in the antibiotic treated group than among patients who do not take antibiotics [6,7]. Ampicillin was one of the first drugs which were connected to skin eruptions in IM.…”

“…The incidence of skin eruption in acute IM is 4.2 to 13% without drug intake [5]. By the frequent use of antibiotics within acute IM, the incidence of skin reactions rises, ranging between 27.8% and 69%; in some past studies for ampicillin even 90% [1,3,6,7]. According to the literature there is no obvious consensus on the cause of skin symptoms, whether a true drug sensitization or only transient immunoactivation develops.…”

“…28 Despite these remarks, it is a promising test for the laboratory diagnosis of drug allergies. 9,42,43 Another test used in research concerns the assessment of serum IL-5 levels secreted by peripheral blood mononuclear cells in the presence of the analyzed drug. 44 This test has a diagnostic sensitivity of 92%, which is higher than that of the lymphocyte transformation test (78%) or of the patch tests (55%).…”

“…10 The incidence of skin reactions due to the oral administration of ampicillin or amoxicillin during an acute episode caused by the Epstein-Barr virus (infectious mononucleosis) is elevated among children and young adults. 9 Familial susceptibility to AR also has been reported, 11 and may be related to different metabolic pathways of the drug, and to the antigenic process.…”
