The purpose of this study is to evaluate the reasonable parameters to predict unstable syndesmotic injuries in ankle fractures. Seventy consecutive patients who underwent preoperative bilateral computed tomography (CT) scans were enrolled. Group A consisted of 20 patients intraoperatively diagnosed with syndesmotic injuries according to an intraoperative stress test and group B consisted of 50 patients who had nosyndesmotic injuries. The tibiofibular overlap (TFO) and tibiofibular clear space (TFCS) were measured using preoperative ankle radiographs. Measuring the anterior fibular distance (AFD), posterior fibular distance (PFD), anterior translation distance (AT), fibular diastasis (FD), anterior-posterior translation (APT), fibular length (FL), and surface area of syndesmosis (SAS) 1.0 and 1.5 which 1.0 cm and 1.5 cm above the tibial plafond was done via preoperative CT scan. The ratio of measurements (Injured/Intact) of the TFO, PFD, APD, and SAS 1.0 showed statistically significant differences. The researchers identified the SAS 1.0 as the most reasonable parameter to predict transfixation using receiver-operating characteristic (ROC) curve analysis. The SAS 1.0 is most valid parameter to predict syndesmotic injuries in this study and these results show that performing a bilateral CT scan on an ankle fracture may provide substantial information in measuring valid parameters.

Introduction: In situ fixation is the gold standard for mild and moderate slipped capital femoral epiphysis (SCFE) cases. The condition is associated with a low percentage of avascular necrosis and chondrolysis.[1] The present study examines the postoperative femoral neck remodelling and implant elongation in cases of therapeutic and prophylactic in situ fixation using the free-gliding screw system.  Aim: The aim of our study was to assess the postoperative femoral neck growth and evaluate the biomechanical evolution and complication’s rate for 19 therapeutic and 11 prophylactic in situ fixations with a free-gliding screw.  Materials and methods: We measured the preoperative and postoperative articulo-trochanteric distance (ATD), alpha angle (α angle) and screw elongation in symptomatic hips and in contralateral hips with prophylactic fixation. We compared the radiographic param-eters of 30 hips.  Results: ATD remains approximately the same for symptomatic cases, whereas it increases for prophylactic fixated hip. Screw elongates in both group with statistically higher value for the prophylactic group. The alpha angle remains pathological in these cases with a mean value of 67.12±4.62°, but decreases for group II. Screw elongates by a mean value of 3.14±2.74 mm for group I and 6.78±8.81 mm for group II. Conclusions: Prophylactic in situ fixation with free-gliding screws does not affect the proximal femoral growth (ATD), and does not decrease the alpha angle significantly. Screw elongates statistically in both groups, but more significantly for group II. For symptomatic hips, the in situ fixation allows the femoral neck to grow with ATD preserved, but significantly less than in group II. The alpha angle decreases but remains pathological.

Background. The study of the absorbing and cytokine-inducing function of phagocytosis is a mandatory starting point in assessing the state of anti-infective protection in children who suffer from frequent respiratory infections. It is important to understand and compare the features of phagocytosis in these children depending on their age. The purpose: to improve the diagnosis of immune disorders in children aged 2–5 and 6–10 years, who often suffer from acute respiratory diseases, by means of a comparative analysis of phagocytosis parameters and serum levels of some cytokines. Materials and methods. Under observation, there were 60 children aged 2 to 10 years who often suffer from acute infectious diseases of the respiratory tract. Two groups were formed: 1) children aged 2–5 years (n = 30); 2) children aged 6–10 years (n = 30). Results. Among patients of the first group compared to those of group 2, an increase in the frequency of low indicators of the phagocytic number (by 36.7 %, p < 0.05), phagocytic index (by 46.7 %, p < 0.05), the index of completion of phagocytosis (by 66.6 %, p < 0.05) and the staphylococcus-stimulated nitroblue tetrazolium test (by 46.4 %, p < 0.05), an increase in the frequency of high serum levels of interleukin-1β (by 33.3 %, p < 0.05), interleukin-10 (by 27.7 %, p < 0.05), a decrease in the frequency of high levels of interleukin-6 (by 16.7 %, p < 0.05). In the children of group 2 compared to those in the first one, high index of completion of phagocytosis was more often registered (by 70.0 %, p < 0.05), as well as more than a twofold increase in the ratio of interleukin-6/interleukin-10. Conclusions. In children aged 6–10 years, more frequent morbidity is due to a cytokine imbalance that was reflected in a more than a twofold increase in interleukin-6/interleukin-10 ratio, which is a negative factor indicating an enhancement of pro-inflammatory processes and a longer damage to the cells of the immune microenvironment.
