BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease with complex and multifactorial pathophysiology, involving elements of barrier dysfunction, alterations in cell-mediated immune responses, IgE sensitization, and environmental factors. This encourages the search for predictors of disease development among both genetic markers and environment. AIM: The aim of the study was to examine if genetic factors of Malassezia recognition, or Malassezia colonization may be related to IgE sensitization or to severity of AD. METHODS: The study included 106 patients with eczema and 103 healthy children. Specific IgE against Malassezia mix (m227) was analyzed in 51 patients using immunochemiluminescent method on the ImmunoCAP 100 (Thermo Fisher Scientific Inc., Phadia, Sweden). Genotyping for rs7309123 in Dectin-1 was performed using Real-time PCR. The level of colonization by Malassezia in the scale samples was determined by a real-time PCR assay. RESULTS: Increased IgE to Malassezia spp. was observed in 29,4% of children with eczema. Higher Malassezia spp. – specific IgE titer positively correlated with severity of AD, age of onset, head–neck type of AD, and a higher total IgE. GG genotype rs7309123 Dectin-1 is significantly more often found in the patients than in the control group, but no correlation with IgE sensitization to Malassezia was found. Malassezia restricta and M. globosa were predominant in patients and controls, with some predominance of M. globosa over M. restricta among patients. CONCLUSION: Sensitization to Malassezia, genetic markers in Dectin-1, and Malassezia colonization of the skin can be tools for studying the gene-environment interactions in the pathogenesis of AD.

Aim: review of modern data of scientific medical literature regarding the prevalence, comorbidity, risk factors, features of pathogenesis, diagnosis, principles of treatment and prevention of allergic diseases in children. In recent years, the number of allergic diseases (ADs) has been increasing, especially among children. Approximately 20% of the world's population suffers from these diseases. Comorbid ADs are quite common. Thus, in 67–85% of children, allergic rhinitis is combined with bronchial asthma. Children with atopic dermatitis are diagnosed with allergic rhinitis in 61,8% of cases, and bronchial asthma in 29,4% of cases. The risk of developing bronchial asthma or allergic rhinitis is higher in children with food polysensitization, and infants and young children with atopic dermatitis have a high risk of developing both food allergy and asthma and allergic rhinitis. According to modern data, most ADs have common causal mechanisms. In the development and progression of asthma and allergic rhinitis and atopic dermatitis, the hypotheses of mechanistic damage of the epithelial barrier and biodiversity, related to each other, are considered. In ADs, mechanisms related to allergen-specific immunoglobulin E and non-allergic coexisting mechanisms play an important role. In addition, views on the formation of the sequence of the development of ADs ("allergic march") from the perspective of multimorbidity and phenotyping are increasingly expanding. Pathogenetic treatment of ADs is aimed at controlling the chronic T-helper 2, caused by the inflammatory process in target organs. Secondary and tertiary prevention of ADs in the early stages improves the quality of life of patients and improves the course of atopic conditions. Conclusions. The prevalence of ADs in modern conditions is pandemic in nature. The hypothesis of mechanical damage to the epithelial barrier is significant in the development of ADs. ADs, in particular in children, can be combined, creating multi- and comorbid conditions, and aggravate the course of these diseases. Timely diagnosis of ADs is important, and a personalized approach to the treatment and prevention of ADs is considered the most rational.

Study of the impact of atopic dermatitis in children on the quality of life of families

Background. Atopic dermatitis is a chronic, relapsing eczema. Although atopic dermatitis occurs mainly in children, it greatly affects the life of the whole family. The purpose was to assess the quality of life of the families of children with atopic dermatitis and to study the correlation between the severity of atopic dermatitis and the quality of life of family members according to the Family Dermatology Life Quality Index (FDLQI) questionnaire. Materials and methods. The study included 104 patients with atopic dermatitis aged 6 months to 17 years (median of 6 [3; 9] years) who were treated at the Allergy Department of the Kyiv City Children’s Clinical Hospital 2. Clinical parameters included age, sex of the patients, disease duration and severity of atopic dermatitis. The latter was assessed by the SCORing for Atopic Dermatitis (SCORAD) tool. Spearman’s rank correlation index was used for analysis of correlation between FDLQI and SCORAD indicators. Р < 0.05 was considered statistically significant. Results. The quality of life of the family of 104 children with atopic dermatitis was assessed using the FDLQI questionnaire. Four parents (3.8 %) reported no impact of the disease on the family’s quality of life, 65 (62.5 %) — a mild effect, 34 (32.7 %) — a moderate impact, parents of 1 patient (1.0 %) noted a very large effect. The average FDLQI was 6.4 ± 2.8. There was a correlation between the FDLQI and the SCORAD severity index (Ro = 0.714, p < 0.01). In the subgroup of children under 4 years of age, a strong correlation between the FDLQI and the SCORAD was found (Ro = 0.789, p < 0.01). Child’s age and disease duration were not associated with the quality of life score (Ro = –0.011, p > 0.05 and Ro = 0.076, p > 0.05, respectively). In the subgroup of children aged 4–17 years, a strong positive correlation of FDLQI and the SCORAD was also determined (Ro = 0.714, p < 0.01). The age of the children and the duration of the disease were not related to the quality of life score (Ro = –0.011, p > 0.05 and Ro = –0.027, p > 0.05, respectively), although in the subgroup of younger children (4–7 years) we noted a tendency towards a greater influence of the disease on the family’s quality of life. Conclusion. In this study, most parents of children with atopic dermatitis reported a mild to moderate impact of the disease on their quality of life. The FDLQI of family members correlated with the severity of atopic dermatitis.
