ABSTRACT.Purpose: To classify a cohort of 62 patients with vernal keratoconjunctivitis (VKC) in immunologic, functional, and epidemiological terms. Methods: A retrospective chart review was conducted to establish the patients' ethnic origin and to ascertain the results of standard in vitro and in vivo testing for atopic allergy. The latter data were compared with tear Phadiatop A , an allergy screening test, in 31 subjects. Further subgroup analyses included methacholine bronchial provocations, serum screening for chlamydial antibodies and epidemiological calculations of the observed prevalence of VKC in the Stockholm area in 1994. Results: Thirty-seven subjects (59.7%) were sensitised to common allergens. No additional allergic subjects were diagnosed with the tear Phadiatop A test. The serology for ocular chlamydial disease was negative. Only 6 out of 17 subjects displayed bronchial reactivity to methacholine of whom 4 had a history of asthma. VKC was clearly more common in individuals with an Asian and African origin. Conclusions: Sensitivity to allergens is a strong determinant for the disease but in a large proportion of the subjects this immunologic abnormality is absent. The varying prevalence of the condition in different ethnic groups indicates a genetic predisposing factor.

A high level of immunoglobulin E (IgE) in cord blood (CB) is well known to be predictive of the development of clinical allergy during early childhood. We have tried to ascertain the source of IgE in cord blood by carrying out a series of measurements of IgE concentrations in arterial and venous CB of newborns and comparing them to IgE concentrations in blood samples from both parents. We found that the concentrations of IgE in arterial and venous CB were almost completely identical in the 131 newborns examined whereas IgD concentrations were significantly higher in arterial than in venous CB. Moderate but significant positive correlations were found between IgE concentrations in maternal blood and CB; paternal blood and CB; and maternal and paternal blood. Our data suggest that CB-IgE is essentially a product of fetal lymphocytes. The positive correlations found between the IgE blood concentrations in the newborns, the mothers and the fathers, is most likely an effect of the amount and nature of the allergenic constituents in the family environment.

The possible causative relationship between exposure to house dust mite allergen (HDMA) and symptoms of vernal keratoconjunctivitis (VKC) was investigated. VKC patients were evaluated for an average of 18 months using patient reports of symptoms, physicians’ observations and assays of sera and tears for total and specific IgE to 10 inhalant allergens common in Israel, including HDMA. Tear and serum samples from a group of 8 patients reacted only with HDMA, and were compared with the tear and serum samples of a group of 11 patients not reacting to any of the above allergens. House dust samples from the homes of two VKC patients in the HDMA-reactive group were examined monthly for a year. While mites were detected throughout the year, they were approximately 3 times more abundant from June through September than at other times. 96.6% of the mites collected belonged to the family Pyroglyphidae; 91.0% were Dermatophagoides pteronyssinus and 5.5% Dermatophagoides farinae. Both the severity of VKC symptoms in HDMA-reactive patients and mite population levels peaked simultaneously in the summer. This relationship was not seen among the non-HDMA-reactive patients. Our results suggest that exposure to HDMA plays an important role in the aggravation of VKC symptoms in HDMA-reactive patients.
