Alcohol, Immunosuppression, and the Lung

Abstract: Bacterial pneumonia is the most common cause of lower respiratory tract infection in immunocompromised populations, including the alcohol-abusing patient. Furthermore, alcoholics are frequently infected with highly virulent respiratory pathogens and consequently experience increased morbidity and mortality from bacterial pneumonia. The resulting increase in health care resource use in these patients represents a significant public health concern. Host defense mechanisms are operant from the nasopharynx to the … Show more

“…One possible explanation is that practitioners taking extra time to think about medication interactions or proper dosing in patients with alcohol abuse since they are relatively immunosuppressed. 25 One possible explanation is that married patients may have been accompanied by spouses who requested the use of newer, albeit more expensive, antibiotics. 26 It is interesting to note that other patient demographics and health status were not associated with inappropriate prescribing practices.…”

“…Considering that ED and heart diseases share similar cardiovascular risk factors, and the wellknown chronic cytotoxic effects of alcohol on general health, 29 hepatic function 30 and immune function, 31 general health might be a mediator between the association of high alcohol consumption and ED. This J-shaped relationship finding might explain why studies have shown harmful effects of heavy alcoholism on sexual function, that is alcoholism entails excessive drinking and carries the increased risk of ED at the tail of the J curve.…”

“…The third mechanism involves antimicrobial peptides, proteins and other molecules secreted by respiratory epithelia Brogden et al, 2003;Hiemstra and Bals, 2004). Ethanol exposure significantly alters at least some of these lung innate responses (Happel and Nelson, 2005) in adults. While each of these innate immunity mechanisms are important in the initial respiratory host defense, their activity in individuals born pre-term with or without the influence of intrauterine ethanol exposure is poorly understood.…”
