Objective: To determine the capabilities of a sonographic examination to evaluate local and systemic hemodynamic disorders in patients with varicose disease. Materials and Methods: This study included 583 patients with varicose veins, among whom 348 had trophic disorders. All subjects underwent lower limb duplex sonography and superficial, perforator, and deep venous diameter measurements, as well as had a calculated velocity for antegrade and reflux flow. Peripheral resistive index (RI) in the arteries, accompanying perforator veins (PVs), was analyzed. In addition, echocardiography was performed on 46 of the participants. Results: A statistically significant difference in the diameter of the great saphenous vein was obtained between class C2 and C4 varicose veins ( P < .05). Similar findings were obtained for the diameter of the PVs and the velocity reflux flow through PVs. Arteries in the perforator bundles demonstrated low RI, which may be attributed to the presence of arteriovenous shunts. Varicose veins were associated with an increased incidence of nonrestrictive type of blood flow through the tricuspid valve. Conclusions: Sonography demonstrated that additional factors rather than hemodynamics may be attributable to the development of venous ulcers. The most informative parameters of cardiac function, in a patient with varicose veins, are the indicators of diastolic function in both ventricles.

Thermodynamic analysis of the complex reduction of metal melts is considered. The proposed analytical method identifies the influence of the weaker reducing agent in amplifying the effect of the stronger reagent. The curves of oxygen solubility pass through a minimum. Analysis of the extremal curves of oxygen concentration in the melt as a function of the content of reducing agents yields a formula for the content of the stronger reducing agent such that the oxygen concentration is minimal. Thermodynamic analysis of the combined influence of aluminum and silicon on the oxygen solubility in Fe-Co melts indicates that the reac tion products may contain both mullite (3Al 2 O 3 ⋅ 2SiO 2 ) and kyanite (Al 2 O 3 ⋅ SiO 2 ). The presence of silicon in the melt intensifies the reducing action of aluminum: slightly when mullite is formed and significantly when kyanite is formed. When kyanite is formed, the curves of oxygen solubility pass through a minimum, whose position depends on the aluminum content in the melt but not on the silicon content. The aluminum content at the minimum declines slightly from iron to cobalt, as for Fe-Co-Al systems. Further addition of aluminum elevates the oxygen concentration. The formation of the compounds Al 2 O 3 , 3Al 2 O 3 ⋅ 2SiO 2 , Al 2 O 3 ⋅ SiO 2 , and SiO 2 is investigated as a function of the Al and Si content in the melt.

The influence of the phospholipid platelet-activating factor (PAF), its cell analogs, and lipid PAF antagonist on the production of superoxide radicals by leukocytes isolated from the blood of healthy and hypercholesterolemia IIA individuals was studied. It was found that endogenous superoxide production level in the leukocytes of hypercholesterolemic individuals more than 4-5 times higher than in the leukocytes of healthy individuals. Exogenous PAF stimulates the superoxide production in the leukocytes of healthy individuals but significantly inhibits the superoxide production in the leukocytes of hypercholesterolemic individuals. The compounds 1-acyl-2-acetyl-sn-glycero-3-phosphocholine (1-acyl-PAF) and 1-alk-1;-enyl-2-acetyl-sn-glycero-3-phosphocholine (1-alkenyl-PAF) only slightly inhibited the endogenous superoxide production in the leukocytes of hypercholesterolemia individuals. However, pretreatment of leukocytes by 1-alkenyl-PAF or PAF-antagonist (1-O-alk-1;-enyl-2-(2;-acetoxybenzoyl)-sn-glycero-3-phosphocholine) results in a 50% inhibition of the PAF-induced superoxide production by leukocytes of healthy individuals. This PAF-antagonist alone or in combination with PAF induces a substantial (65-70%) inhibition of superoxide production in the leukocytes of hypercholesterolemic individuals. It is concluded that superoxide production by leukocytes of healthy individuals and especially by leukocytes of hypercholesterolemic individuals is process that depends on PAF or PAF-like lipids.
