Combining drugs with different mechanisms of action results in additive and/or synergic effects providing thus opioid-sparing strategies in pain management. This studydetermines the antinociceptive interaction after intravenous co-administration of ketamine and lidocaine in rat models of acute and inflammatory pain and evaluates the effect of the combination on motor activity. Thermal pain is accessed by tail-flick. Inflammatory pain is induced bycompound Freund adjuvant (CFA) into the right hind paw. Mechanical allodynia is assessed with von-Frey filaments. Motoreffects are evaluated by rotarod apparatus.The profile of interaction is assessed by Isobolographic analysis. Ketamine and lidocaine, alone or in combination, dose-dependently ameliorate acute thermal pain and mechanical allodynia triggered by CFA. The dose of combinationthat reach 20% or 50% of the maximum possible effect is lower than the expected for an additive interaction suggesting, thus,synergism. The interaction indexes are 0.001 and 0.029 for thermal and inflammatory pain, respectively. Ketamine alone but not in combination reduces the motor performance in the rotarod at 50% antinociceptive dose. Combined ketamine and lidocaine synergistically attenuates thermal and inflammatory pain. Therefore, smaller doses of both drugs retain their analgesic efficacy and are associated with lower alterations in motor activity.

Aim The aim of this study was to compare the extent of analgesia in terms of duration of analgesia and requirement of postoperative analgesics with preemptive analgesia and transversus abdominis plane (TAP) block in patients undergoing inguinal hernioplasty. Materials and Methods Ninety male patients aged between 18 and 65 years of American Society of Anesthesiologists grades I and II undergoing open inguinal hernioplasty were randomly allocated to three comparable groups of 30 each. Control group (group C) received normal saline; preemptive group (group P) received inj. paracetamol, inj. diclofenac, and inj. Lignocaine; and TAP group (group T) received TAP block with 20 mL of 0.25% bupivacaine at the end of surgery. Parameters recorded were verbal numerical rating scale (VNRS) at rest at 4, 12, 24, and 48 hours and VNRS on movement at 12, 24, and 48 hours; heart rate and mean arterial pressure at 0, 4, 12, and 48 hours; time for first analgesic request; and total doses of analgesics consumed at 24 hours. Results Demographic data were comparable. VNRS on movement and number of doses of analgesics consumed at 24 hours were statistically less in group T (4.80 ± .761 hours); time for first request of analgesia was longer and this group had better patient satisfaction scores compared with other groups. Conclusion Both groups P and T provided better VNRS scores compared with group C. TAP block provided better VNRS scores on movement at 24 hours, lesser analgesic requirement, and better patient satisfaction score compared with other groups.

Проведен анализ литературных данных по особенностям применения ларингеальной маски (ЛМ) в лапароскопической хирургии с целью обеспечения эффективной и безопасной анестезии. ЛМ - это разновидность надгортанных воздуховодных устройств, которые являются альтернативой интубации трахеи и имеют ряд преимуществ с большим профилем безопасности. Возможность и целесообразность использования ЛМ при робот-ассистированных ивидеолапароскопических оперативных вмешательствах остается спорным вопросом в связи с опасениями возникновения осложнений - интраоперационной регургитации и легочной аспирации на фоне повышенного внутрибрюшного давления при искусственно созданном карбоперитонеуме. В данном обзоре приведены преимущества применения ЛМ 2-го поколения при лапароскопии, конструкция которых позволяет обеспечивать эффективную вентиляцию с достижением адекватного дыхательного объема и оксигенации. Однако необходимо соблюдать четкие правила установки ЛМ и контроля за безопасностью их использования при анестезиологическом обеспечении лапароскопических вмешательств. The analysis of literature data on the peculiarities of the use of laryngeal mask (LM) in laparoscopic surgery was carried out in order to ensure effective and safe anesthesia. LM is a type of supraglottic airway devices that is an alternative to tracheal intubation, and it has a number of advantages with a large safety profile. The possibility and feasibility of using LM in robotic-assisted and videoassisted laparoscopic surgical interventions remain controversial due to the fear of complications intraoperative regurgitation and pulmonary aspiration on the background of increased intraabdominal pressure with artificially created carboperitoneum. This review summarizes the advantages of using the 2nd generation LMs in laparoscopy, the design of which lets to provide efficient ventilation with the achievement of adequate tidal volume and oxygenation. However, it is necessary to follow the clear rules for installing LMs and monitoring the safety of their use in the anesthetic provision of laparoscopic interventions.
