Feasibility of complex minimally invasive surgery in neonates

The aim of this study was to assess the feasibility of complex minimally invasive surgery in neonates (and infants <5 kg). A systematic search of databases was performed. Inguinal herniotomy and pyloromyotomy were specifically excluded. Various operations were assessed for operative times, conversion rates, complications and centres performing the procedures. From 479 abstracts, 50 papers were identified describing 1,215 patients. Common operations performed were fundoplication (n = 473), operations for ovarian pathology (n = 151) and oesophageal atresia repair (n = 123). Other not-so-common operations included Kasai procedure (n = 61), Ladd's procedure (n = 55), bowel atresia surgery (n = 45) and congenital diaphragmatic hernia repair (n = 41). Mean operation times varied from 43 min for fundoplication to 209 min for Kasai procedure, with other operation times falling in between the two extremes. Likewise, conversion rates varied from 0.2% for fundoplication to 15% for congenital diaphragmatic hernia, with other conversion rates falling in between. Complications ranged from 0 to 67%. About 70-94% of the cases were performed in six pioneering centres. Fundoplication and operations for ovarian pathology have low complication and conversion rates and appear to be well suited to the average-trained paediatric laparoscopic surgeon. More complex procedures are feasible and safe, but technically demanding and need further expertise.

44Эндоскопическая хирургия у новорожденных и детей грудного возраста до недавнего времени ка-залась невыполнимой задачей. Трудность освоения минимально инвазивной хирургии у новорожден-ных и младенцев заключается не только в миниа-тюрных размерах объектов, подлежащих коррекции, но и в дефиците знаний, технологий и практических навыков, необходимых для производства операций, которые способствуют анатомо-функционально-му восстановлению аномально развитых органов. Другим сдерживающим фактором продвижения эндохирургических методов лечения у новорожден-ных и детей грудного возраста является недостаток сведений о физиологических эффектах минималь-но инвазивной хирургии в этой возрастной группе. Истинное воздействие нового лечебного подхода на организм маленького ребенка по-прежнему остает-ся неопределенным. Несмотря на то что минималь-но инвазивная хирургия постепенно становится привилегированной техникой у младенцев, лапа-ротомия и торакотомия в 56,0-91,4% случаев все еще остаются самыми популярными действующими подходами для лечения хирургических заболеваний новорожденных [1].Торакоскопическое лечение атрезии пищевода стало новой эпохой в истории лечения атрезии пи-щевода. Торакоскопия или видеоассистированная грудная хирургия (Video-Assisted Thoracic Surgery) позволяет выполнить ряд вмешательств на органах грудной клетки без использования торакотомии.Кроме минимизации травматичности и улучшения косметических результатов, торакоскопические операции полностью отвечают международным этическим требованиям в детской хирургии: «вы-бирать из равно эффективных методов те, которые наименее сложны, наименее болезненны, наименее дорогостоящи» [2].Прогресс эндоскопии у новорожденных на про-тяжении последних 15 лет конвертировал ряд от-крытых операций в видеоассистированную грудную хирургию, в том числе и при атрезии пищевода. Основоположниками эндохирургической коррек-ции эзофагеальной атрезии в 1999 г. стали детские хирурги из США [3] T. Lobe (рис. 1) и S. Rothenberg (рис. 2).
