Adverse events during CT colonography for screening, diagnosis and preoperative staging of colorectal cancer: a Japanese national survey

Abstract: • The colorectal perforation rate during preoperative-staging CTC was 0.028 %. • The perforation rates for screening and diagnosis were 0.003 % and 0.014 %, respectively. • The perforation risk is significantly lower in screening than in preoperative staging. • Eighty-one per cent of perforation cases did not require emergency surgery. • Use of an automatic colon insufflator can reduce the risk of bowel perforation.

“…In 2015, a Japanese national survey of 147 439 CTC examinations [58] revealed lower perforation rates of 0.014 % overall, albeit with a higher rate when CTC was used for preoperative staging (0.028 %); and a much lower rate for screening RECOMMENDATION ESGE/ESGAR suggest CTC in patients with high risk polyps undergoing surveillance after polypectomy only when colonoscopy is unfeasible. Weak recommendation, low quality evidence.…”

“…During screening or diagnosing colonoscopies, the overall adverse event rate has been reported to be around 2.8 per 1000 acts [6]. Even with computed tomographic (CT) colonography, a recent study including 431 Japanese centres with 147,439 CT examinations showed 0.014% of colorectal perforations [7]. In addition, tissue biopsies are also time-consuming.…”
