73Dermatoloji Polikliniğine Başvuran Hastalarda Oral Mukoza Hastalıklarının Araştırılması: 1670 Hasta Kapsayan Bir Çalışma Ö ÖZ ZE ET T A Am ma aç ç: : Birçok dermatolojik hastalığa oral mukozal bulgular eşlik eder. Oral mukozal lezyonlar (OML) bazen tanımlanamamış bir hastalığa doğru tanı koymada yol gösteren tek ipucu olabilir. Türkiye'de OML üzerine yürütülmüş oldukça az sayıda çalışma mevcuttur. Biz de bu çalışma ile genel Türk toplumunda OML'nin prevalansını ve bunu etkileyen faktörleri ortaya koymayı hedefledik. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Bin altı yüz yetmiş hasta (950 kadın, 720 erkek) bu çalışma kapsamında incelendi. Hastaların yaş, eşlik eden hastalıklar, ilaç hikayesi, sigara ve alkol kullanımı, dolgu, kaplama ve protez gibi dental muayene bulguları, dermatolojik hastalıkları ve mevcutsa OML'ı kaydedildi ve ayrıntılı istatistiksel analizler yapıldı. B Bu ul lg gu ul la ar r: : OML'nin genel prevalansı %37,8 idi (kadın=%34,8, erkek=%41,6). Paslı dil en sık karşılaşılan OML idi (%6,7), bunu fissüre dil (%4,4) ve Fordyce granülleri (%3,9) izlemekteydi. Paslı dil erkekler, orta yaşlı erişkinler (35-54 yıl) ve 20-39 sigara/gün sıklığında sigara içenlerde istatistiksel olarak anlamlı oranda daha fazla tespit edildi. Alkol kullanımı ile OML görülme sıklığı arasında istatistiksel olarak anlamlı kabul edilebilecek herhangi bir ilişki saptanmadı. Venöz göllenme, protez stomatiti ve glossit ileri yaş (65 yıl ve üzeri) ile istatistiksel anlamda ilişkili bulundu. Bu çalışma grubunda kandidiyazis ve Behçet hastalığı erkeklerde, glossit ise kadınlarda istatistiksel olarak anlamlı oranda daha yüksek tespit edildi. Retinoids, namely tretinoin and isotretinoin, are metabolically active derivates of vitamin A and used as therapeutic agents. Tretinoin is part of the standard therapy for patients with acute promyelocytic leukemia. Isotretinoin is applied to children with medium and high risk neuroblastoma. Further, both isomers are frequently administered to patients with acne and other skin or mucosal disorders.Isotretinoin usage in dermatology has been associated with several ocular side effects. We report on the first pediatric patient with neuroblastoma who suffered from acute transient bilateral vision loss due to optic neuritis induced by systemical administration of isotretinoin. Therefore toxic optic neuritis needs to be considered as a potential adverse effect of isotretinoin therapy and weighed carefully against therapeutic benefits.By quantum mechanical calculations it could be shown that 13-cis/trans-isomerism of tretinoin has only minor influence on the molecular shape and electrostatic potential suggesting that the biochemical interactions as well as adverse effects for tretinoin and isotretinoin might be similar.

Adverse effects of isotretinoin: A retrospective review of 1743 patients started on isotretinoin

Abstract: Other than the two oral contraceptive failures, there were no serious adverse events recorded during this review period. Isotretinoin is a very effective medication with a low adverse-effect profile when used at lower doses.
