Advantages of cone beam computed tomography (CBCT) in the orthodontic treatment planning of cleidocranial dysplasia patients: a case report

Abstract: Our aim was to discuss, by presenting a case, the possibilities connected to the use of a CBCT exam in the dental evaluation of patients with Cleidocranial Dysplasia (CCD), an autosomal dominant skeletal dysplasia with delayed exfoliation of deciduous and eruption of permanent teeth and multiple supernumeraries, often impacted. We think that CBCT in this patient was adequate to accurately evaluate impacted teeth position and anatomy, resulting thus useful both in the diagnostic process and in the treatment pla… Show more

“…In addition, the precision of radiographic diagnosis had been enhanced by the introduction and ongoing improvement of three-dimensional imaging techniques [45], creating better conditions for selective mobilization of teeth [46]. This progress is also reflected in the case presented here; CBCT was of enormous benefit for the diagnosis and set the stage for successful treatment -five supernumerary teeth would have gone undetected without this technology.…”

“…La chirurgie orthognathique par ostéotomie de Lefort ou par repositionnement mandibulaire peut alors améliorer l'esthétique et la fonction lorsque le traitement s'achève avec une arcade mandibulaire complète associée à une arcade maxillaire rétrusive et à une importante béance antérieure, ou lorsque le traitement est plus tardif et nécessite un fort accroissement de la dimension verticale d'occlusion [10].…”

“…At present, the treatment plan involves the removal of retained deciduous teeth and persistent supernumeraries [14,15]; a closed technique simulating natural tooth eruption is preferred [16]. After removal of retained deciduous and supernumerary teeth, various orthodontic methods can be used to bring impacted teeth into their correct positions, such as removable braces with elastic tractions of 60-90 g [11], fixed transpalatal arches, thin-wire braces with elastic tractions and skeletal anchorage [14]. The common upper jaw hypoplasia implies the need to use headgear and/or expanders in orthodontic treatment [17].…”

“…Radiological examination, and CBCT in particular, is invaluable in the diagnosis of CCD as it allows for accurate localization of persistent teeth and an assessment of bone structure which usually has dense bone trabeculae. CBCT is preferred in young patients due to lower radiation doses than in conventional tomography and excellent image quality [11,12].…”
