Advanced Robotic Surgery of the Parapharyngeal Space: Transoral Robotic Styloidectomy in Eagle Syndrome

Abstract: Eagle syndrome (ES) is characterized by symptomatic elongation of the styloid process or ossification of the stylohyoid ligament causing irritation and inflammation of the trigeminal, facial, glossopharyngeal, and vagus nerves. The use of robotic surgery has been accepted as a first-line treatment for some head and neck squamous cell carcinomas but not for styloidectomy. The aim of this article is to document our experience with a transoral robotic approach to treat ES and to present the outcomes of 6 patients… Show more

“…Another interesting point regards the comparison with the main results obtained in other Centers by transoral robotic surgery (TORS), a relatively new procedure in the treatment of parapharyngiomas. Its indications are substantially similar to those mentioned above for the transoral approach [ 33 , 34 , 35 , 36 , 37 , 38 ]. The procedure is based on the use of the Da Vinci Surgical Robotic System, a robotic device that combines great operational precision with easy use and handling.…”

“…19 Among the complications, a single case of suture dehiscence 4 days after surgery was described. 18 Regarding pain control, discomfort resolution, or any other Eagle syndrome-related symptoms, we noted that 10 patients (59% of the total) experienced complete relief from their symptoms.…”

“…One patient (5.9%) complained about sporadic right side chewing discomfort. 18 The last six patients (35%) were divided by the author (Fitzpatrick et al 16 ) into a group of five patients with a 'meaningful' improvement of the symptoms and a group with still an improvement but defined 'non-meaningful' in one patient. The 'meaningful' group included improvements from a moderate to a near complete resolution.…”

“…In some patients, partial styloidectomy is preferred to complete resection 20 . The transoral approach is a viable alternative to styloidectomy 29,30 . Piezosurgery styloidectomy, endoscopic or robotic, is considered outstanding by some authors 20,30,31 .…”
