Adhesive/Dentin Interface: The Weak Link in the Composite Restoration

Abstract: Results from clinical studies suggest that more than half of the 166 million dental restorations that were placed in the United States in 2005 were replacements for failed restorations. This emphasis on replacement therapy is expected to grow as dentists use composite as opposed to dental amalgam to restore moderate to large posterior lesions. Composite restorations have higher failure rates, more recurrent caries, and increased frequency of replacement as compared to amalgam. Penetration of bacterial enzymes,… Show more

“…Finally, it should be noted that this investigation did not involve the demineralized dentin matrix. The size of interfibrillar spaces (~ 20 nm) may physically exclude phase-separated structures, i.e., resin globules > 10-20 µm, as suggested by our previous work (Spencer and Wang, 2002;Spencer et al, 2010).…”

“…MDPC-23 cells (3310 4 ) were seeded on the pulpal side of the dentin disks (0.28 cm ). In order to accomplish that, the IVPCs were placed in an inverted position (pulp side up) into the compartments of 24-well plates (CO-STAR 3595, Corning Incorporated, Corning, NY, USA) and maintained in an incubator with 5% CO 2 and 95% air at 378C for 48 hours to ensure adherence of cells. After this time, the IVPCs were carefully removed from the compartment and returned to the same compartment with the occlusal side up to receive the treatment solutions.…”

“…1,2 Therefore, the resistance of the adhesive interface components against degradation determines its stability and durability. 1,[3][4][5] Collagen fibrils that are not encased by monomers during the bonding procedure, 6,7 as well as those exposed by the polymer degradation over time, are highly susceptible to enzymatic hydrolysis.…”

“…The classification was based upon whether or not a separate etching agent was used [16,17]. An etch-and-rinse system is characterized by removing the smear layer by using an etching agent [18], and a self-etching system is characterized by eliminating the technique-sensitive rinsing step and reducing the operation time [19].…”
