Failed Back Surgery Syndrome (FBSS) is persistent pain and disability following lumbar laminectomy which is associated with decreased quality of life and disability and has been reported in up to 40% of the patients undergoing lumbar laminectomy. Several approaches have been introduced to reduce the rate of the FBSS. Among these, applying anti-adhesive barrier gels have been studied with interest with controversial results. The aim of the current study was to determine the effects of anti-adhesive barrier gels on functional outcome and recurrence of patients undergoing lumbar disc surgery. We searched databases including EMBASE, PUBMED, Web of Science, Scopus, Cochrane Library, and scholar databases until November 2019. To assess the heterogeneity across included studies was used Cochran's Q and I-square (I 2 ) statistics. Standardized mean difference (SMD) and 95% CI between were used to estimate pooled effect sizes. Out of 4507, 10 clinical trials found to be appropriate for current meta-analysis. The pooled results of included clinical trials indicated that adhesion barrier gel significantly decreased leg pain (LP) (SMD ¼ À0.31; 95% CI, À0.60, À0.03; P ¼ 0.032; I 2 : 59.2%) among patients with lumbar disc herniation surgery. Back pain (BP) (SMD ¼ À0.03; 95% CI, À0.23, 0.16; P ¼ 0.734; I 2 : 40.2%), and Oswestry disability index (ODI) (SMD ¼ À0.11; 95% CI, À0.27, 0.05; P ¼ 0.178; I 2 : 0.0%), were not significantly affected following adhesion barrier gel application. Application of adhesion barrier gel in single level lumbar disc surgery is associated with deceased leg pain. However, its application does not affect the low back pain, disability and gate. Further, larger randomized clinical trials are required.

В нашем исследовании представлен анализ 63 литературных источников из баз данных PubMed, EMBASE, Cochrane Library, eLIBRARY, наиболее полно отражающих патогенетически обоснованную профилактику формирования эпидурального фиброза в до-операционном, интраоперационном и послеоперационном периодах. Objective: To identify the morphological patterns of development and prevention of experimental epidural fibrosis based on the shape of the dural sac in the area of laminectomy. Methods: Male Wistar rats aged 4-5 months underwent laminectomy at the level of L6-S1. The animals were divided into two groups with 7 animals per group. In the main group (Group A) the animals were treated with Antiadhesin® gel applied to the wound to prevent the development of epidural fibrosis, while in the group of comparison (Group B), the gel was not applied. Another 7 healthy animals were used as a control group. The animals were taken out of experiment on the 28th day. Histological slides stained with hematoxylin-eosin were used to measure the width and length of the dural sac, its area, perimeter, and calculate its deformity index. Results: In group B at the level of laminectomy, the ratio of the width to length of the dural sac was significantly higher than in the control group (p<0.05), mainly due to the enlarged width, while the length did not change significantly. In group A these parameters did not differ from the control group, which indicated that application of Antiadhesin® prevented formation of a connective tissue scar which could compress the dural sac and cause its deformation. In group A in the area of surgical intervention, granulomas with Pirogov-Langhans cells were detected, both in the soft tissues and in the lumen of the spinal canal. These findings in perioperative tissues might be related to biocompatibility and indicate their reaction to a gel as a foreign body. Conclusion: In clinical practice optimal materials for the prevention of epidural fibrosis are missing. There is an obvious need for further research in order to obtain implants capable of inhibiting and reducing the formation of epidural fibrosis.
