Background and aims: Acanthamoeba spp. as opportunistic microorganisms are widespread in a wide range of environmental sources in the world and may cause severe diseases in immune-deficient patients. The aim of this study was to determine the prevalence and genotypes of Acanthamoeba spp. in hemodialysis patients in Shahrekord county of Chaharmahal and Bakhtiari province. Methods: In this cross-sectional study, 79 oral swabs were collected from the mouth of patients with chronic renal disorder undergoing hemodialysis from June to October 2018. The samples were then cultured on non-nutrient agar and examined by Giemsa staining, polymerase chain reaction (PCR), sequencing, and tolerance testing methods. Results: Morphological investigations indicated that 30.4% (24/79) of the subjects were infected with some free-living amoebae (FLA), and the PCR showed that only 3 samples contained Acanthamoeba spp. The sequencing of the latter samples demonstrated that these isolates belonged to T2 and T4 genotypes. After performing the assay tolerance test, 2 of these 3 isolates were related to T4 genotypes representing a high pathogenic potance. Conclusion:The infection of hemodialysis patients with some of Acanthamoeba spp. particularly, the T4 genotype should be considered important for these patients. Therefore, it is recommended that dialysis machines and dialysis units in hospitals be checked and disinfected periodically.

Background Dialysis is a method for removing excess fluid and uremic waste from the body of chronic renal-failure patients. In patients with CRF, blood dialysis stops death. Hemodialysis prevents the death of patients with chronic renal failure. Various factors can affect the survival of patients undergoing dialysis treatment. The current study aimed to determine such factors.Methods This descriptive-analytic study examined 252 patients undergoing dialysis from 2010 to 2016 in all of hospitals in Bandar Abbas- Iran, whose data had been recorded in the dialysis unit or it was possible to get their files. The study used Lasso Regression in Cox's proportional hazards model in case with multi variables, background diseases, Body Mass Index,…… The collected data was analyzed using SPSS software and through installing glmnet and survival packages in R software.Results In this study, 35 (13.9%) death events and 217 (86.1%) censored events were observed. The mean survival time of 252 patients was equal to 10.93 ± 7.82 years and the median survival time of these patients was calculated to be ten years. Based on the results of the Cox model under the penalized method of Lasso, The risk of mortality for patients with diploma degree was by 49% lower than that of the illiterate group (HR = 0.51). The risk of death for unemployed patients and farmers was equal to 0.66 (HR = 1.66) and 0.29 (HR = 1.29), respectively, higher than that of employees. Another significant factor in this study is the duration of each dialysis session. The risk of mortality has been reduced by 0.34 (HR = 0.66) for a single increase during dialysis.Conclusions Apart from congenital and underlying diseases which early diagnosis will prevent kidney failure, the risk of mortality will be reduced by training behavioral issues in patients undergoing dialysis such as dialysis at predetermined hours and times and appropriate nutritional behaviors.

Background: The adequacy of haemodialysis in patients of type 2 diabetes mellitus with chronic kidney disease stage 5 depends on several clinical as well as laboratory parameters. Previous studies from Western literature have identified several clinical and laboratory markers for predicting adequacy of dialysis. There is a dearth of literature regarding the same in Indian patient populace. Authors aimed to find correlation, if any, between glycemic control and adequacy of dialysis in this cohort of patients.Methods: A set of 200 patients of type 2 diabetes mellitus who have undergone hemodialysis at a tertiary care hospital were included in the study. Random blood sugar (RBS), Glycated hemoglobin (HbA1c) were measured at admission. After 4 hours of dialysis, the urea reduction ratio (URR) and Kt/V was measured for each patient. The correlation coefficient as well as linear equation of the association between these variables were calculated. Standard statistical method and software were used in the process.Results: The study revealed a linear negative correlation between the variables RBS, HbA1c and URR as well as Kt/V. This suggests the importance of pre dialysis glycemic control in patients undergoing hemodialysis.Conclusions: Authors formulate the hypothesis that glycated hemoglobin and random blood sugar at admission correlate well with the outcome and adequacy of dialysis in patients of stage 5 chronic kidney disease undergoing haemodialysis. Good glycemic control (HbA1c <6.5 % and RBS <120 mg/dL) have shown to be important predictive markers of adequate dialysis. The hypothesis needs to be tested with a larger study.
