Adenocarcinoid tumor of the colon arising in preexisting ulcerative colitis

Abstract: Patients with ulcerative colitis are at increased risk of developing adenocarcinoma of the colon. The authors describe a patient whose colonic neoplasm demonstrated histologic characteristics of both an adenocarcinoma and a carcinoid tumor and which was pathologically identical to a appendiceal adenocarcinoid. Because individual tumor cells stained positively for both mucin and argentaffin granules, the histologic picture is unique among the malignancies seen in patients with ulcerative colitis and cannot be e… Show more

“…1 Only a few cases of carcinoid tumor arising in ulcerative colitis have been reported, [2][3][4][5][6][7][8][9] and in some of these reports it has been noted that in addition to the inflammatory changes characteristic of ulcerative colitis, there was evidence of gut endocrine cell hyperplasia in areas of damaged bowel.…”

“…In particular, whether these lesions represent a logical step in the tumorigenesis to a carcinoid malignancy has not yet been determined. [2][3][4][5][6][7][8][9] This case report presents a patient with ulcerative proctitis who was identified on biopsy as having rectal microcarcinoids.…”

“…4 Two patients with GCC in the colon or rectum with peritoneal seeding were given adjuvant chemotherapy. 4,5 In the present case, multiple pulmonary recurrence was observed 15 months after the patient's initial surgery, despite curative resection. The patient was about to start chemotherapy with a modifi ed FOLFOX-6 regimen, which is widely adopted as a treatment for recurrent colorectal adenocarcinoma.…”

“…15,16 Similarly, to our knowledge no case of GCC of the colon or rectum has been diagnosed preoperatively, although a carcinoid tumor of the sigmoid colon was suspected after colonoscopic biopsy in one case. [5][6][7][8][9][10] The present case was diagnosed as rectal adenocarcinoma based on the preoperative colonoscopic biopsy fi ndings, because signet-ring-like cells with copious mucin had invaded the lamina propria of the biopsy specimen. Histopathologically, GCC closely resembles signet-ring cell carcinoma; however, retrospective examination of the specimen from our patient suggests that GCC might be histologically differentiated from signet-ring cell carcinoma by the fact that the cancer cells tend to cluster rather than infi ltrate diffusely.…”

“…The authors suggest that carcinoids develop from neural crest cells which have migrated to embryonic gut endoderm. 25 By analogy, in the thyroid, endodermally derived stem cells may give rise not only to follicular cells but also to parafollicular cells.…”
