Actions in small companies to promote occupational health: the case of the food and beverage sector Agir em empresas de pequena e média dimensão para promover a saúde dos trabalhadores: o caso do setor de alimentos e bebidas Ada A. Nascimento 3 AbstractObjectives: To describe working conditions in small companies in the food and beverage sector and to assess the health and the work ability of the selected workers. Methods: Social security data and records from periodic medical examinations were analyzed. For the observations of the work conditions, we used a data sheet created specifically for this study to apply the Short Ergonomic Assessment.For the survey, the Work Ability Index (WAI) and the Nottingham Health Profile (NHP) were used. Results: The following risk factors and distress-generating situations were observed: manual transport of loads; work performed while standing and in production lines; improvised workbenches; hazardous facilities; and poorly maintained machines. Most of the workers were male with a mean age of 32 years. The mean WAI score was 43 (SD=4.25), suggesting good work ability. Musculoskeletal diseases were the conditions most frequently diagnosed and the most frequently self-reported symptoms. According to the NHP, the workers' general health status could be considered good. Conclusions: Despite the relevance of small companies in the national economy, many of them are family-based and lack the capital needed to invest in infrastructure and productive processes. Furthermore, the lack of knowledge about occupational health and safety puts employees at risk. Positive results were discerned following the present study that included investigation, training and intervention to support small companies, which are generally excluded from broader social actions. The results could be documented through the longitudinal monitoring of the companies.Key words: occupational health; work capacity; perception of health; working conditions; small companies. ResumoObjetivos: Descrever as condições de trabalho em micro e pequenas empresas do setor de alimentos e bebidas e avaliar a saúde e a capacidade para o trabalho dos trabalhadores selecionados. Segundo o PSN, o estado geral de saúde dos trabalhadores pode ser considerado bom. Conclusões: Apesar da relevância das empresas de pequena dimensão na economia nacional, a base familiar e o reduzido capital explicam os baixos investimentos na estrutura física e nos processos produtivos. Ainda, a falta de conhecimento sobre segurança e saúde no trabalho gera situações de risco para o empregado.Vislumbram-se resultados positivos, que poderão ser documentados por meio de monitoramento longitudinal, após esta experiência que aliou investigação, formação e intervenção para apoiar essas empresas, geralmente excluídas de ações sociais mais amplas.Palavras-chave: saúde do trabalhador; capacidade para o trabalho; percepção de saúde; condições de trabalho; pequenas empresas.

INTRODUCTIONThe aging of the population is a worldwide phenomenon, highlighting the significant increase in the population aged 70 years and older. In 2009, the results of the National Household Sample Survey indicated a total of 11.3 million in this age group. The decrease of the children and youth and the consequent increase in adult population and elderly is related to the continued decline in the fertility and mortality rate combined with an increased life expectancy (1) .Simultaneously, the most prevalent diseases among elderly are also changing, with a sharp decline in the morbidity and mortality from infectious and contagious diseases and an increase in chronic non-communicable diseases (CNCDs) (2) . With aging, the probability of acquiring CNCD increases while functional capacity and body reserves decrease, leading to a greater susceptibility to these diseases and other health problems (3) . Thus, there may be a direct association between the demographic and epidemiological transition.According to the Brazilian Institute of Geography and Statistics, in 2008, the most prevalent diseases in seniors were: high blood pressure (53.3%), back pain (35.1%), rheumatoid arthritis or rheumatism (24.2%), heart diseases (17.3%), diabetes mellitus (16.1%) and other diseases (20.9%) (1) . The CNCDs may result in losses in the overall health and functionality of the elderly, negatively affecting their quality of life, including the standard and quality of sleep (4) .Aging by itself can bring changes in the usual pattern of sleep, such as quantitative reduction of deep sleep stages and increase in the light sleep stages, nocturnal sleep disruption, increased sleep onset latency, reduced duration of nocturnal sleep, and more frequent transitions between the sleep stages during the night. The prevalence of sleep-disordered breathing and disorders related to increased nocturnal myoclonic activity are also more common over 60 years of age, so that the complaints of insomnia symptoms are most frequent among the elderly (4)(5) .There are four essential symptoms for the diagnosis of sleep disorders and insomnia: difficulty falling asleep, difficulty staying asleep, early awakening and non-restorative sleep. Various international classification manuals for sleep disorders shared these criteria, including the Research Diagnostic Criteria of the American Academy of Sleep Medicine (RDC), the International Classification of Sleep Disorders (ICSD-2) and the International Classification of Diseases Organization of the World Health Organization (ICD 10) (6) .Changes in sleep patterns and insomnia symptoms in seniors may occur as a consequence of CNCDs. Other prevalent diseases in this age group may influence sleep quality, among them, we highlight depression, bladder weakness and neurological problems, such as Parkinson's disease and stroke (7)(8) . | Background: According to the International Classification of Functioning, Disability and Health (ICF), the participation component is considered to be the most complex component characterized in existing instruments that assess quality of life related to health (HRQoL). Questionnaires such as the Nottingham Health Profile (NHP) and the Stroke-Specific Quality of Life (SS-QOL) are currently used to evaluate the participation of individuals in life activities after stroke. However, because participation and HRQoL are different constructs, it is unclear whether these instruments are best suited to this evaluation. Objective: To investigate whether the NHP and SS-QOL are suitable instruments for assessing the participation component of ICF. Method: A cross-sectional study was conducted in which a sample of 35 individuals completed the SS-QOL and NHP. The study correlated the total score on the questionnaires with the scores on items that evaluate the participation component of the ICF. Both analyses used the Spearman correlation coefficient (r). Results: There was a statistically significant, positive and strong correlation between total score on the SS-QOL and NHP and the score on specific components related to participation (r=0.8, p=0.001 and r=0.9, p=0.001, respectively). Conclusions: Both the NHP and the SS-QOL show strong correlations between the total score and the score on items that assess the participation categories of the ICF. However, the SS-QOL questionnaire appeared to be the most complete for this assessment because it evaluates more distinct domains and contains the largest number of items related to participation.
