Acute Tubular Necrosis Following Interferon-Based Therapy for Hepatitis C: Case Study with Literature Review

Abstract: Background/Aims: Interferon treatment of malignant or viral diseases can be accompanied by various side-effects including nephro-toxicity. Methods: We report on a 68-year-old Caucasian male who received dual therapy with pegylated interferon 2a plus ribavirin for chronic hepatitis C. Results: After three months of antiviral therapy, the patient developed acute kidney failure (serum creatinine up to 6 mg/dL) with mild proteinuria (500 mg daily) and haematuria. Immediate immunosuppressive therapy with high-dose … Show more

“…Ribavirin renal toxicity has not been documented and is not probable in monotherapy[65,66]. Nevertheless, by analogy with the ribavirin apoptotic activity observed in K562 leukemia cells, potential tubular toxicity has been hypothesized[65,67].…”

“…Recombinant IFN given for treatment of HCV may exacerbate proteinuria in some patients with underlying glomerulopathies. 323 Regardless of the regimens used (IFN-based or DAAs), antiviral treatment of HCV-related glomerular disease has limitations. First, the impact of antiviral therapy on the longterm outcomes of kidney disease remains uncertain.…”

“…Nephrotoxicity is unusual during treatment; however, there are case reports of acute tubular necrosis induced by PEG [2]. There are also reports of proteinuria induced by this regimen, which disappear after the cessation of treatment.…”
