Acute Pericarditis as the First Manifestation of Familial Mediterranean Fever: A Possible Relationship with Idiopathic Recurrent Pericarditis

Abstract: A 56-year-old man was admitted to our hospital due to periodic episodes of acute pericarditis. These episodes occurred monthly along with a high fever and elevation of the C-reactive protein (CRP) level. The patient became afebrile and his CRP level decreased following the administration of a non-steroidal antiinflammatory drug. A mutation analysis revealed the heterozygote of the familial Mediterranean fever (FMF) gene (E84K, G304R). This finding confirmed our diagnosis, and we treated the patient with colchi… Show more

“…While pericarditis typically presents simultaneously with other FMF symptoms,[ 8 ] it has been reported as the only manifestation of the attack. [ 6 7 11 13 14 15 ] In those cases, recurrent pericarditis which are refractory to the typical treatments raised the suspicion of FMF requiring further genetic studies to confirm the diagnosis.…”

“…She was compound heterozygous for E148Q and L695A. Yoshioka et al [7] described a 56-year-old man who had a high fever, CRP level, and periodic episodes of acute pericarditis. The patient was treated with colchicine, became afebrile and CRP level decreased.…”

“…[3,4] Recurrent pericarditis as the initial finding has been reported in a few case with FMF. [5][6][7] Here, we report a patient with suspected FMF and recurrent pericarditis who responded to colchicine therapy.…”

“…While most of the pericardial symptoms occur in conjunction with other FMF manifestations, solitary pericardial involvement has been described in the literature [24][25][26]. A diagnosis of FMF should be suspected in patients with recurrent episodes of pericarditis refractory to treatment, requiring further genetic analysis for the accurate diagnosis.…”
