Acute pericarditis: An unusual presentation of an exacerbation of ulcerative colitis

Abstract: Acute pericarditis is an unusual complication of ulcerative colitis. In some rare cases, it is the only extraintestinal manifestation of the underlying colitis, and may even precede the development of bowel symptoms. Less rarely, pericarditis may be the presenting feature of an exacerbation of ulcerative colitis. Such a case is reported to illustrate the important, but uncommon, association of these disorders, and to discuss the implications for management.

“…This hypothesis was supported by a quick remission of the clinical picture after the drug therapy was suspended. We should as well consider the wide diffusion of 5‐aminosalicylic acid treatment in IBD and the quick reaction of pericarditis to steroidal treatment, independently from drug intake, 8,51 as was seen in one of our patients, affected by pericarditis and cardiac tamponade. There may be some cases related to the use of mesalazine or similar drugs, that may cause lupus‐like syndromes 52–54 .…”

“…Crohn’s disease (CD) may present various extra‐intestinal manifestations such as osteoarticular, hepatobiliary, nephric, cutaneous, and ocular involvement 1 . Cardiac involvement in the course of inflammatory bowel disease (IBD) is considered very rare, and reports of endocardial 2 and valvular 3 involvement, myocarditis, 4,5 pericardial effusion 6–8 and bradyarrhythmias 9 are few and sporadic.…”

“…Myopericarditis has not always been associated with the activity of the bowel disease [1]. It can even precede the development of bowel symptoms [2]. Myopericarditis has also been described as a side‐effect of the treatment of IBD [3].…”
