Acute Management of Auricular Hematoma

Abstract: This retrospective case series is used to describe a refined technique for the aspiration and drainage of auricular hematoma that is simple, cost-effective, and allows for rapid recovery. Patients, all high school males participating in competitive wrestling, were enrolled voluntarily after risks and benefits were discussed, and consent was obtained. Criteria for enrollment included acute auricular hematoma of at least 2 cm in size with occurrence no greater than 3 weeks before presentation, and no overt signs… Show more

“…The current management of auricular hematoma mainly depends on its size; while smaller hematomas should be aspirated; larger ones require open surgical drainage. Further studies are required to elucidate the efficacy of post-drainage interventions including splinting or bandaging ( 19 , 20 ). New techniques using fibrin glue and an 18-gauge catheter with a compression dressing are recently proposed.…”

“…New techniques using fibrin glue and an 18-gauge catheter with a compression dressing are recently proposed. Cost-effectiveness and efficacy of these management options remain to be confirmed by further studies ( 20 , 21 ).Use of local flaps in soft tissue defects, initial realigning of the helical rim and anatomical repair of ear lacerations should be considered in management of acute ear trauma. In cases of ear amputation, staged reconstruction or replantation techniques should be used ( 22 ).…”

“…En el caso presentado, el hematoma incluyó varias regiones del pabellón auricular como el antihélix, incluida su rama inferior, fosa escafoidea o navicular y concha, siendo este aspecto infrecuente, pero que en el caso descrito si se presentó 8 . El hematoma auricular se presenta como una tumoración de aspecto azulado, blando, fluctuante y doloroso a la palpación, que desfigura la morfología normal del pabellón debido al borramiento de sus relieves 7,8 .…”

“…The perichondral blood vessel tears as a result, and blood accumulates between the perichondrium and cartilage. The hematoma obstructs blood flow, mechanically causes cartilage infection and necrosis 12 . The presence of necrosis around the cartilage will further inhibit the normal ossification process of the cartilage that lives above the defect 13 .…”
